Southeast Tennessee Development District Area on Aging & Disability Department - Senior Health Coordinator

The Southeast Tennessee Development District is seeking a Senior Health Coordinator to provide resources and support to consumers and/or family members of the programs sponsored by SETAAAD/SETD. This position will be responsible for the planning and coordination of community health promotion programs in our 10 county region that focus on brain health, disease self - management, and falls prevention. In addition the coordinator will provide case management to individuals living alone with dementia and assist with outreach, awareness activities and project development to meet the needs of older adult. The ideal candidate will have excellent communication skills (both written and verbal), be able to maintain confidential information, and have outstanding organizational and computer skills. Candidates must be able to establish and maintain positive and effective working relationships with internal/external contacts and partners and has the ability to work independently with minimal supervision as well as part of a team. A Bachelor’s Degree in social work, psychology, gerontology, sociology, counseling, or equivalent degree. Experience in social services, gerontology, or closely related field is preferred.
