POSITION SUMMARY: Provides nursing and ACT case management services in the community and provides physician support for consumers with severe mental illness. Assists in planning, linking, advocacy, coordination and monitoring; to assist consumers in gaining access to health services, financial assistance, employment, education, social services and natural supports. Assesses needs, develops and monitors Person Centered Plans and documents treatment in an accurate and timely manner. Provides video or audio assessments after hours on a rotated schedule between all team members.
EDUCATION: Associates Degree from an accredited nursing education program.
EXPERIENCE/KNOWLEDGE: One to two years’ experience as a Registered Nurse and experience working with individuals with mental illness preferred. Must possess excellent computer and interpersonal skills, punctuality, ability to organize and manage several projects/tasks simultaneously, and work independently.
LICENSURE/CERTIFICATIONS: Licensed by the Michigan Department of Commerce or Professional Regulation as a Registered Nurse. Must have reliable means of transportation for home visits and maintain safe driving record.
