Case Manager Position

The Lansing Area AIDS Network is seeking a full time Medical Case Manager to join our Clinical Team in providing services to diverse populations living with HIV.  Ideal candidates should possess a Bachelors in Social Work degree, however, other combinations of education, work experience, and training will be considered.

Candidates should have experience and understanding of case management principles, knowledge of community resources, experience working with chronically ill populations, a commitment to diversity including varied cultural and experiential backgrounds, and a demonstrated ability to work effectively with diverse populations and as members of a case management team. Responsibilities will include providing medical or non-medical case management services; assessing and determining client needs, developing individualized care plans including medical case management, referral for medical, mental health, and substance abuse services as necessary, medication and treatment adherence, housing assistance, and support services.   All applicants must have clear verbal and written communication skills, strong interpersonal and team skills, and demonstrated computer skills.   Experience in the areas of HIV care services, public health, mental health, or substance abuse services is preferred.  HIV Case Management Certification training will be provided.
