Change Management Advisor - Institute for Hope

Summary: 


Works in partnership with the Institute for H.O.P.E.TM and Population Health Innovation leaders to support identified high priority organizational projects and efforts, by applying change management, process improvement (PI) methodologies and related concepts to create executable plans. Creates and implements PI strategies that maximize program efficiency and effectiveness through optimal employee workflow and improved utilization of business and information technology. Focused on aligning, enabling, and improving existing workstreams and projects, and guiding the process of change with new work and projects across people, processes, systems and technology. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.

 

 

Responsibilities:


1. Supports the initiatives by:
a. Formulating, implementing, and evaluating strategies to align with the strategic goals.
b. Preparing, presenting, and assisting in team/committee presentations and progress reports.
c. Initiating and facilitating collaborative relationships amongst team members to facilitate strong program impact and social determinants of health (SDOH) aggregate and individual outcomes.
d. Facilitating development of Epic workflows for SDOH information and reporting.
e. Maintaining compliance with regulatory standards related to SDOH billing.
f. Establishing and maintaining effective work relationships with multidisciplinary teams and community-partners related to SDOH.

2. Oversees, promotes, and participates in the implementation of PI strategies for SDOH metrics and Institute for H.O.P.E. programs by:
a. Assisting team decision making using proven PI and Quality methodologies and tools for new SDOH metrics.
b. Assessing existing processes and identifying PI opportunities.
c. Monitoring SDOH and quality data to identify gaps in program effectiveness and collaborating with program leadership on methods to improve barriers.
d. Working to integrate value-based contract, quality, and SDOH metrics into overall quality assessment and PI initiatives.
e. Participating in the development and implementation of indicators that measure the process and outcomes of care.
f. Compiling, initiating and maintaining PDSAs (Plan, Do, Study, Act cycle) for all PI projects initiated.
g. Developing, presenting and coordinating related continuing education programs and presentations related to PI, SDOH, poverty and social justice to employees and community-based partners.
j. Assisting teams and departments in setting quality performance measurements and tools as it relates to SDOH for data monitoring, analysis, reporting and presentation.

3. Leverages process and project management tactics to include: change management, lean six sigma, and other quality improvement tools and methodologies to develop strategies based on a situational awareness of the details of the change and the groups being impacted by the change to ensure impact goals and outcome metrics are met.

4. Partners with internal and organizational stakeholders to deploy a set of actionable and targeted change management and PI plans - including communications, sponsor engagement, coaching, training, resistance management, and monitoring sustainability; integrates change management activities into project plans and leads change management workstreams.

5. Conducts program assessments, evaluates results, and develops plans to ensure business operations are prepared for upcoming changes based on lessons learned and redesigned processes.

6. Advises on development and assists in delivery of a comprehensive communication plan ensuring consistent messages are being delivered to the right audience at the right time in the right channel and engaging format.

7. Identifies and analyzes potential risks, points of resistance, performance gaps, and develops specific corrective action plans to mitigate the risks.

8. Consults with sponsors and leaders to develop the vision for change, understand underlying issues and conducts organizational assessments and project re-design analyses.

9. Enhances professional growth and development by keeping current with trends and practices through participation in continuing education courses, professional organizations, seminars and workshops; reading current literature and maintaining professional contacts within the community.

10. Displays sensitivity to and understanding of various cultural, ethnic, racial, and socioeconomic backgrounds.

 

 

Qualifications:


Required:
Bachelor’s degree in Organizational Leadership, Management, or related field, or any equivalent combination of education, training, and experience in addition to the experience stated below.
Four to five years’ experience successfully implementing change management processes, best practices, operational change initiatives and related communication efforts.
Knowledge and application of PI techniques to include: Organizational Change Management methodologies; Lean Six Sigma, value stream mapping, cost-benefit analysis and return on investment decision making.
Familiarity with project management approaches, tools and phases of the project lifecycle.
Familiarity with SDOH concepts, the impact on health outcomes, and leading practices to mitigate their effects.
Ability to influence others and be flexible and adaptable; able to work in ambiguous situations.
Results oriented and motivated by the opportunity to take on new initiatives with a sense of urgency.
Excellent communication skills (written, verbal, interpersonal and presentation).
Knowledge of Microsoft applications (e.g., Word, PowerPoint, Excel).
Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds.

Preferred:
Master’s Degree in Business, Communications, Change Management, Process Improvement, Organizational Transformation or related field.
Lean Six Sigma certification.
Project Management Institute (PMI) certification.
Change Management Methodology certification.
Experience working within a Healthcare system.
Highly developed communication skills with expertise in producing organization-wide communications and development of a communication strategy.

Physical Demands:
Will need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating.
Will need to remain still for extended periods, including sitting and standing.
Ability to communicate in face-to-face, phone, email, and other communications.
Ability to read job related documents.
Dexterity to use computer.
