Join our team of compassionate and competent professionals as a Housing Specialist at Behavioral Health Network!

Provide high quality, person-centered care coordination related to tenancy sustaining and pre-tenancy supports and needs.

Provide telephonic and in-person Care Coordination to assist individuals to find and maintain a safe affordable place to live.

Additionally, the Housing Specialist provides services to members in the community to help overcome barriers impacting their engagement (member homes, health clinics, collateral providers, and supportive services).

Communicates and negotiates with the landlord and/or property manager regarding the member’s needs, including disability and reasonable accommodations.

The Housing Specialist meets program specific requirements for productivity, member contacts, documentation, and work attendance.

Who are we?

The Care Management program provides team-based, comprehensive medical and behavioral Care Management (CM) services for children, youth, and adults in Western Massachusetts! CM teams are staffed by professionals including Registered Nurses, master’s Level Behavioral Health Clinicians, and paraprofessional Care Coordinators. BHN partners with insurers and integrated primary care practices to provide whole health management.

Behavioral Health Network is a growing non-profit community behavioral health agency that has been providing services to children, adults, families and communities in Western Massachusetts since 1938. BHN provides comprehensive, outcome-driven behavioral health care. What are we looking for in a Housing Specialist?

Qualifications/Requirements

  • 1 year of work experience with supportive housing programs.
  • 1-2 years of work experience in a related behavioral health, substance use, human services and/or medical field (required).
  • Knowledge of supportive housing programs and community resources.
  • Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, social determinants of health, and care coordination.
  • Familiarity with Internet, email systems, and electronic health records (EHRs).
  • English-Spanish speaking (Required).
  • High school diploma required; Community Health Worker Certification and/or bachelor’s degree preferred.
