Seeking a Special Needs Care Coordinator to assist families of children with special health care needs. •Meets with all active Special Needs patients every 6 months – 1 year to complete an updated Care Plan. •Faxes updated Care Plan to the school. •Sends the completed Care Plan to the family once the primary physician signs the plan of care. •Scans Care Plan into PCC and attaches it to the patient’s chart within 30 days of updated Care Plan. •Completes training biannually to stay up-to-date on treatment options for special needs children. •Returns all patients and specialists office calls within 48 hours of the business day received. •Coordinates care with specialists, families, and schools. •Schedules appointments with specialist for Special Needs patients and documents in orders in the patients chart. •Attends IEP meetings with families as requested. •Attends group meetings to assist families in understanding their care. •Educates families: dental techniques for special needs children, asthma medications and use, appropriate use of the emergency department, child safety, catheter devices, and feeding techniques. •Contacts special needs families and schedules appointments for flu vaccination. •Obtains authorizations for patients with Tricare, Aetna, Blue Choice and all HMO Medicaid plans and documents authorization information in orders. •Supervises any assistants or volunteers not employed with Palmetto Pediatrics working in Special Needs department. •Assists special needs families with transition of care when transferring out of state or to an adult provider.
