Summary:
Demonstrates proficiency in overseeing the daily operations of the case management office functions. Works closely with the department leadership to organize and coordinate office procedures, goals and objectives, and promote established policies. Supports the progress of short and long-term goals and objectives for the department with data collection and reporting, includes various quality and performance improvement initiatives that operationally impact the organization.

Responsibilities:
1. Manages Case Management office/telephone reception. Fields calls for Director, Case Managers and Social Workers from nursing units, Insurance Companies, Home Health Agencies, DME Companies, physicians, other facilities, etc.2. Communicates daily assignments and scheduling needs to the department staff in order to manage daily workflow with regards to volume and/or assignment changes.3. Assists department leadership by completing data entry, monthly dashboards, and other document preparation.4. Maintains key data reporting and supports processes that are critical for daily department operations, which includes obtaining the necessary data, preparing reports, and presenting current findings to department leadership and staff. This includes hospital readmission data, community coalition efforts, referral information, workflow efficiency, etc.5. Assists department leadership with the completion of monthly staff schedules.6. Prepares department productivity and staffing reports on a monthly basis.7. Coordinates department projects and fund raisers for the Department for different organizations/charities supported by the organization.8. Prepares data for specified reports from LaserArc, HBO, and other systems daily for department leadership.9. Maintains a transportation log to validate monthly transportation expenses incurred by the case managers and/or social workers.10. Responsible for printing and distributing reports for the Interdisciplinary Team weekly meeting.11. Responsible for the collection and data entry maintenance of discharge information worksheet data for utilization within the department.12. Maintains current listing of Home Health agencies and SNF facilities for distribution to patients in compliance with the Balance Budget Act of 1997. Also maintains a sitter list for distribution to patients.13. Responsible for accuracy of Time and Attendance bi-weekly (payroll) reports and annual time log maintenance.14. Responsible for preparing budget reports and maintaining expenditures within the department allocations. Also monitors the department crisis fund and reports funding status to leadership and staff routinely.15. Maintains and monitors office supplies and equipment for NOMC Case Management Offices.16. Maintains policy and procedure manual as policies are changed or updated.17. Serves as Information Systems liaison for Department, attends other educational classes that benefit the department.18. Assists Department Director/Coordinator with preparation of educational materials used in orientation for new employees, interns and physicians.19. Serves on Task Force Teams and organizes activities and distributes information pertinent to these teams.20. Generates all travel document requests for seminars and other educational travel for the department.21. Schedules and coordinates meetings for department leadership and all monthly department meetings, includes responsibility for transcribing, distributing, and filing of minutes.22. Keep Case Management staff informed of meetings, prepare calendar of events that pertain to staff and keep social workers, LPN’s and RN’s informed of upcoming education to acquire CEU’s.23. Responsible for filing and maintaining departmental records and distribution of department mail on a daily basis.24. “Follows North Oaks Health System’s Compliance Programs and Federal and State Regulatory guidelines.”25. Other duties as deemed necessary and appropriate.
