The Advanced Practice Provider will serve as a Full Time Pediatric Inpatient/Outpatient Specialist, providing care to pediatric patients, birth to 21 years, within the Upstate Pediatric and Adolescent Center and Golisano Children’s Hospital at Upstate.  This clinician will be responsible to care for patients in our outpatient setting, which is an NCQA Patient Centered Medical Home.  Clinical responsibilities include hospital follow ups, acute care visits, continuity visits and other patient specific ambulatory visits.  Additionally, this clinician will work with the Pediatric Hospitalist teams, in the inpatient setting at Golisano Children’s Hospital, serving patients from the Canadian border to the Pennsylvania border, and providing care for children admitted to the hospitalist service with a wide variety of diagnoses.  This is a unique opportunity for a provider to provide continuity from the inpatient service to the outpatient office.

Responsibilities include providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care, assessing & evaluating patients, ordering appropriate laboratory testing & diagnostic studies, arranging for referrals, testing, consultation & therapeutic services, and maintaining outstanding relationships with families, referring physicians & all members of the health care team.

NYS licensed/registered NP or PA required. Master’s Degree as a Nurse Practitioner or Physician Assistant & current National Certification consistent to the clinical practice also required. CPR Certification required at the time of appointment.

Pediatric experience preferred.
