Our Kennewick Aging & Disability Resource Center is opening a full time position (40 hours/Week, Monday-Friday 8-5, all major holidays paid) for a Nurse Care Coordinator. This position coordinates services designed to improve the health of high needs, high risk clients for the Health Home Program.

Responsibilities include in-home assessment, care planning & monitoring of client status, and implementation and coordination of service, as well as health teaching, skin observation, and referrals to medical providers. Experience and Training: Licensed RN in WA State, Graduate of accredited School of Nursing and two years of nursing service, including one year of direct patient care in a community setting, experience working on cross disciplinary teams, meeting & working with people in homes & other medical & community settings. Must maintain 45 CEU's every 3 years. $4,290 - $4,515 per month Health/Dental/PERS Benefits, Comparable to County and State Package Closes 4:00 pm March 6, 2018
