Job Summary:

The Family Medicine Social Worker's primary responsibility is to function as a consultation and referral service to address the psychosocial needs of the patient population served by both outpatient Family Medicine clinics located downtown and at the community campus at Upstate Medical University. Through the use of a wide range of interventions and treatment modalities, the Social Worker contributes to the achievement of optimum health, satisfaction and cost-effective care by addressing the behavioral, social and environmental issues which may impede care.  The ideal candidate would possess knowledge of social determinants of health and community resources as well as strong written and verbal communication. Experience with multidisciplinary teamwork in a healthcare setting is preferred. Upstate Social Workers perform other relevant duties as assigned including back-up coverage, occasional on-call and holiday coverage and support for other social workers to meet operational needs.

Minimum Qualifications:

MSW from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience required.

**New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.**

Preferred Qualifications:

Experience in a hospital and/or healthcare environment preferred.

Work Days:

Monday-Friday, 8:30 am -5:00 pm, occasional on-call/holiday coverage on rotation based on operational needs.
