Job Summary:

In this position, the Emergency Department Social Worker provides comprehensive psycho-social assessment, management and support to at risk patients and families at Upstate Medical University Hospital. Knowledge of social determinants, of health and drivers of utilization are preferred. Duties will include consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patients and families. Candidate shall learn to initiate and implement advance directives. The Social Worker will actively manage patients through multidisciplinary collaboration, coordination, assessment, identification of barriers to care and referrals for services to meet individual's health needs. The Social Worker rounds on their assigned patients with the multidisciplinary team and participates in quality meetings on their assigned unit. Candidate must possess strong written and verbal communication skills, have knowledge of available community resources, participate in departmental meetings/in-services, education, and quality improvement activities. Upstate Social Workers perform other relevant duties as assigned including back-up coverage and support for other social workers to meet operational needs.

Minimum Qualifications:

MSW from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Preferred Qualifications:

 

Work Days: Days 7:00 am - 3:30 pm with every other weekend rotation. Includes occasional on-call/holiday coverage on a rotation based on operational needs.
