Full Time Days, No weekends, serving our Montcalm county community.

Job Summary:

In collaboration with the entire interdisciplinary team, utilizes clinical judgement and expertise to coordinate care management interventions. Develops strategies to ensure cost effective, quality care outcomes to meet a patient/family’s end-of-life care needs. Assess, plans, monitors and provides relationship centered care principles and holistic philosophy assuring the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, sacred death.

Spectrum Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. Essential Functions:

• Works in close collaboration and communication with physicians and other members or the healthcare team to minimize duplication of effort, promote teamwork and ensure seamless coordination of services within and between teams and across campuses.
• Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Acts as a patient/family advocate to ensure their desires are recognized and followed to the greatest extend possible.
• Identifies and responds to any situations representing risk to the patient, their family, the organization and/or the community.
• Assists with patient evaluation and treatment, actively participate in care conferences as appropriate.
• Evaluates patient’s and significant other’s educational needs and develops and implements appropriate teaching strategies or makes appropriate referrals.
• Partners with community agencies to coordinate care across the continuum to offer options, tools, and resources to all patients the opportunity to achieve his or her desired goals.

Qualifications:

• Master’s Degree Social Work – Required
• Licensed MSW in State of Michigan – Required Upon Hire
• Michigan Driver’s License – Required Upon Hire

• 1 year of relevant experience – Preferred
