ABOUT THIS POSITION:
UW Medical Center - Northwest has an outstanding opportunity for a PER DIEM SENIOR SOCIAL WORKER to join our Care Coordination Department. This position will work day shift, 8 hours per shift, with variable days each week.

The Senior Social Worker serves as a member of an integrated, multidisciplinary team that is focused on the assessment, support and post-acute disposition of patients and their families.  This position will seek opportunities to create improvements in resource utilization, length of stay and appropriate discharge planning criteria.  These duties will be performed in accordance with, and in adherence to, the mission, vision, and values of UW Medicine.
