Work as part of a dynamic interdisciplinary team in an innovative new program: Bridging Older Adult Care Program (Back Up). This program provides a high degree of medical management to elderly patients as they recover from illness at home following a hospitalization. The goal of the program is to prevent rehospitalization and successfully transition the patient back to a community provider.
This position includes a combination of remote telephonic/EMR work as well as face to face patient visits. You will work closely with a physician, nurse practitioner, and LPN admissions coordinator. The program currently serves patients living in Kent County.

• Assesses internal and external referrals to identify patient/significant others’ needs, level of intensity, insurance benefits and other patient resources.
• Develops plan of care and makes recommendations to PCPs, specialists and other members of the health care team regarding care management strategies, identifying strategies to maximize continuity of care across the continuum.
• Assesses the educational needs of patients, families and members of the health care team and develops and implements appropriate teaching strategies or makes appropriate referrals.
• Communicates and collaborates with patient/significant others/providers/payers to coordinate services that improve access to appropriate services across the continuum of care and which promotes optimal health in a cost-effective manner.
• Measures patient care outcomes, interprets reports and analyzes data trends for groups of patients.
• The care manager position is for the Byron Center Geriatrics Home Based Primary Care Program. Someone in this role will work an average of 50% in a home office doing telephonic care and will also require home visits with patients in the West Michigan area. Candidates will use own vehicle to travel and see patients in their homes.

• Bachelor's Degree or equivalent in Nursing or a healthcare related field required
• BSN must be obtained within 7 years for those who start in this RN role on or after January 1, 2017
• 5 years of relevant experience clinical nursing and or case management/managed care or related field
• Registered Nurse (RN) - State of Michigan Upon Hire required
• Basic Life Support (BLS) - AHA American Heart Association or ARC American Red Cross Ambulatory required upon hire

Preferred Qualifications

• Case Manager, Certified (CCM) - CCMC Commission for Case Manager Certification Upon Hire preferred
