Baystate Health, western Massachusetts’s premier healthcare provider, and home to Baystate Medical Center (BMC), the region’s only level 1 trauma center and pediatric trauma center is looking for a Nurse Coordinator to join our Adult Emergency Department!

DEPARTMENT DESCRIPTION:

BUSIEST ED IN MASSACHUSETTS AND ONE OF THE BUSIEST EDS ON THE EAST. Experience diverse pathologies and acuity as we serve over 122,000 adult and pediatric patients annually. Our 72,000 square foot ED is equipped with 94 private rooms, separate pediatric ED, separate behavioral health rooms and top-of-the-line technology. Providers are engaged in the department process. Staff committees are set up to implement change and create dynamic teams that work well together. BMC is a major referral care center and the only Level 1 trauma center and pediatric trauma center in the area.

Clinical (nurse) Coordinator: Develops/facilitates systems approach to care delivery for a given patient population across care continuum. Assumes leadership role in planning and implementing clinical initiatives that improve quality, efficiency and cost of care delivery. Partners closely with multidisciplinary team and identified physician leaders to optimize patient outcomes and evidenced based practice. Reports to nursing or service line director.

Job Responsibilities: 

1) Partners with identified medical leaders to develop and implement expert clinical care delivery systems for a defined patient population or service.

2) Demonstrates outstanding clinical and service delivery and utilizes critical thinking, problem solving and expert knowledge to identify and facilitate systems improvements with a strong focus on health promotion, risk reduction and population-based health care.

3) Assumes leadership role in planning and implementing clinical initiatives and quality improvements that demonstrate effective positive progress in key performance metrics such as patient satisfaction, LOS, cost/case and staff engagement or others as defined by program goals

4) Collaborates with inter-professional team members to enhance outcomes across the continuum of care by utilizing, critiquing and/or conducting research to promote evidenced based practice.

5) Identifies high risk patient care issues and trends and implements interventions to achieve optimal/benchmark quality outcomes.

6) Assumes leadership role in population based clinical strategic planning and participates on key committees related to that planning

7) Establishes collaborative relationships with regional providers ( primary care, post acute) and local community organizations to advance population health across region

8) Develops and provides clinical education to direct care clinical staff, cross continuum inter-professional team members as well as patients and families.

9) Participates in community education and awareness campaigns to advance population based health initiatives.

10) Knowledgeable about health care /payment reform and its impact on given population. Participates in creating novel delivery models to meet changing regulations and mandates.

11) Provides post discharge follow up and education through telephone/telemedicine

SCHEDULE:

  • Location: Baystate Medical Center, Springfield MA
  • Dept: Adult Emergency Department
  • Hours: Full Time, 40 hrs
  • Day Shift

QUALIFICATIONS: 

  • Licensed as a Registered Nurse in the state of Massachusetts
  • Current American Heart Association Health Care Provider CPR certification
  • Minimum of five years acute care experience
  • Bachelor’s Degree required; Master’s Degree preferred
