The Triage Registered Nurse (RN) works with hospital Operations Center staff, care transitions, nursing, Center for Patient Flow Management, ED, PACU, and providers to ensure patients in assigned Cleveland Medical Center units are progressing towards goals of care in a time-appropriate fashion by monitoring, engaging, and solving for barriers to progression, throughput, discharge, or health system flow. The Triage RN will

  • work with the Manager/Patient Flow Leads/Resource RNs/Leadership to hold providers accountable for inpatient throughput best practices in their assigned units to achieve organizational goals through standardized processes while maintaining a professional, respectful, and collaborative approach with all stakeholders.  
  • assists CMC Operations Center leadership by collaboration with medical staff and nursing leadership, care team members, Care Transitions, nursing, social work, therapy, Utilization Management (UM), and Clinical Documentation Integrity (CDI) to develop, implement, and continuously improve methods to optimize use of hospital services in a supportive and synergetic manner.

Essential Duties Include:

  • Eliminates waste in patient flow by coordinating services among nursing, ED, PACU, UM, CDI, bed management, EVS, and Care Transitions
  • Support standard pathways and protocols to reduce complications, minimize waste,  optimize length of stay (LOS), and improve throughput.
  • Assists in designing and implementing strategies to reduce avoidable hospital readmissions, facilitate objective and evidence-based next site of care leveling, value-focused, and criteria driven next site of care patient choices
  • Analyzes data to identify trends, outliers, educational needs, and improvement strategies to increase early discharge, decrease LOS, improve throughput, improve patient outcomes, and enhance the patient experience. 

Shift:  Full time (40 hours/week), Monday - Friday, no weekends or holidays

  • 2+yrs in frontline clincal care or operational leadership preferred
  • 1+yr RN experience required
  • Graduate of an accredited nursing program required, BSN preferred
  • RN licensure in the State of Ohio required
  • Understanding of all levels of care including ED, ICU, inpatient, observation
  • Clinical knowledge of case management, utilization management and care transitions preferred
  • Stong interest in patient flow, patient progression, throughput & care transitions
