Commonweatlh Place is located in East Syracuse and is a residential treatment facility for alcohol and drug patients.  Service includes medical and bio-psychosocial assessment and treatment planning and daily therapy program.  Therapy program includes individual and group therapy, Chemical Dependency education and education on related topics, activities of daily living therapy, medical follow up and continued care planning

 

The position of the registered professional nurse (RN) within the Chemical Dependency Treatment Service Line will be responsible to play a vital role within the multidisciplinary team caring for the chemically dependent patient seeking treatment at Commonwealth - Crouse Hospital.  The professional registered nurse will have a baseline knowledge of Chemical Dependency treatment and have an understanding of  the continuum of care that is available to the patient in treatment at this level of care and post discharge. The registered professional nurse is responsible for the coordination and administration of all patient care per the protocols of the Article 28 facility, the attending Medical Director & NP as well as the Consulting Psychiatric MD and team.  The registered professional nurse is responsible for the intake screening of all potential/actual patients, all nursing assessments/interventions (initial & daily) and medication administration/reconciliation.   The registered professional nurse will be accountable for providing patient education on a multitude of disease processes/concepts as a direct effect of chemical dependency on the patient population  (eg: HIV/AIDS, Hepatitis, smoking cessation, diabetic management, cardiovascular health, exercise and diet, mental health & behavior modification) as well as to patient families,  in both the individual and group settings.  The registered professional nurse attends daily staffing to assist with the multidisciplinary team in the development/modification and update of all individual patient treatment plans, as per NYS OASAS regulations.  The registered professional nurse is the sole provider of communication of a patient's medical presentation to the clinical staff at Commonwealth Place. The registered professional nurse is both a provider of care as per NYS Nursing practice act and a resource in the inpatient facility to all patients and staff (including dietary, counseling, security and administration).  The registered professional nurse will display cultural competency and apply this to all patient care initiatives. 

 

JOB REQUIREMENTS:

 

Required:IF HIRED OR LICENSED AS A REGISTERED NURSE PRIOR TO 1/1/2017:

Graduate of an accredited Nursing degree.  Currently licensed as a Registered Professional Nurse in New York State.  Minimum of two (2) years Medical/Surgical nursing experience and 100 hours of addiction or mental health education and/or training (completion of mental health/psych rotation at an accredited school of nursing may count toward this requirement).  Valid New York State driver's license is required.  Basic Life Support (BLS) certification required at time of hire or within 2 weeks after hire. Advanced Cardiac Life Support (ACLS) and/or Pediatric Advanced Life Support (PALS) certification must be obtained within one (1) year of the hire date, if deemed necessary for the position based on the scope of the assigned role. Neonatal Resuscitation (NRP) certification for Registered Nurses working in the Kienzle Family Maternity Center must be obtained by the end of orientation.
