Summary

Family Ways is a partnership with communities to supporting pregnant, parenting families, and children.  There are three specific communities of focus services will be provided to: U.S. born Black/African American, the American Indian/Alaskan Native, and the Native Hawaiian/Pacific Islander communities. The Nutritionist will be a consult and resource for team members and conduct care coordination with other team members and referral sources. The Nutritionist will also foster strength-based practice with client and families. They will also carry a case load to provide client and family services. Team members will be upheld to Family Ways Guiding Principles: leading with racial equity, respect, participant-centered services with strength-based practice, collaborative decision making, accountability, and continuous improvement in their services and programming. 
 
This position will train and consult with team members and work closely with individuals and families that identify with the communities of focus in a variety of settings, including home visits. This position will utilize effective teaching and coaching skills, collaborate with others, and apply communication skills that are inclusive and inviting, especially during difficult situations.  In addition, this position will contribute to continuous improvement and quality of services and engage others to contribute to improvements in services and team processes. 
 
This position is situated in the Community Health Services Division in the Parent Child Health Program. Staff may report to work at one or more community-based organizations or county offices as part of their work locations within King County. This is a represented position at 1.0 FTE. Workdays and hours may vary depending on community events, client/family need and staff coverage.  Some weekday evenings and weekend work will be required, as well as the ability to drive and travel to multiple locations throughout King County.

Job Duties

Training and consultation

  • Conduct individual sessions to assess client’s nutritional needs using anthropometric, biochemical, clinical, and dietary information; implement and document care plan.
  • Screen for client health needs and connect to services in a timely manner.
  • Coordinate care with team members and other health care providers and make referrals to appropriate services; provide and document case management, as appropriate.
  • Provide technical assistance and nutrition consultation to other members of the multidisciplinary team; may provide in-service training for staff.
  • Act as liaison with food access and subsidy programs.

Direct care and coordination
  • Partner with client/family to develop care plan that may include support to navigate social and health systems and support to connect to community resources 
  • Conduct educational and informational learning for staff, clients, community groups and other professionals.
  • Respond to calls, emails, or requests in a timely manner. 
  • Promote and/or market sound, accurate, culturally appropriate educational materials, and information.
  • Collaborate with client/family, community, and county programs to support access to services and referrals.
  • Foster and coordinate community and agency partnerships and attend community events.
  • Help assess needs for education for client/families and other staff.
  • Assist in program and educational materials development, as it relates to nutrition content.
  • Implement and evaluate nutrition education materials.
  • Conduct/arrange for culturally relevant and linguistically appropriate presentations for targeted audiences.
  • Implement and initiate processes to ensure public and community involvement.
  • Maintain and update a library of training and reference materials.
  • Gather and assemble data.
  • Perform other duties as assigned. 

Partnership and Community Engagement
  • Develop and present information to clients individually, in classes or to community groups on nutrition, food preparation, safe food handling and related topics.
  • Participate in the collection and analysis of data to identify populations at risk and participate in research and improvement efforts related to health care delivery.
  • Participate collaboratively with the community to assess and address health issues, including the development of health programs.
  • Develop and implement nutrition standards for focused populations such as prenatal and high-risk patients.
  • Foster relationships with community organizations 
  • Support expanding partnerships and program participation. 
  • Collaborate, coordinate, and assist with other County programs and community-based organizations to advance racial equity and social justice. This may include active participation in development of racial equity/anti-racism trainings or resources.  

Leadership 
  • Engage actively and consult with team members in program goals/objectives, especially those related to nutrition. 
  • Be aware and conscious of budgetary and operating conditions for the program. 
  • Develop and assist participant-centered, strengths-based practices. 
  • Apply trauma recovery and healing. Prioritizing racial equity and social justice in collaborative work in the teams and with other county programs.
  • Work collaboratively with evaluation team and electronic medical record support staff. 
  • Collaborate and share decision making with team members fostering and creating space for authentic and respectful dialogue.
  • Experience being an active, productive contributor within a team environment. 
  • Demonstrated skills and abilities working collaboratively with client and families, community-based groups, service providers, other staff, and programs.

Licensing, Certification and Other Requirements
  • Washington State Driver’s License 
  • Current Registered Dietitian  registered with the Commission of Dietetic Registration (RD)
  • Must be a Washington State Certified Dietitian or have an application in process and be able to obtain within 30 days
  • National Provider Identification (NPI) must be obtained within the first 30 days of employment
  • A current BLS CPR certification and AED training required by start date. CPR/AED Heartsaver courses do not meet this requirement. 
  • Able to successfully complete the department credentialing process prior to hire and maintain credentialing for the duration of employment.

Work Schedule
The regular workweek is 40 hours, Monday through Friday.  This role is FLSA non-exempt and is eligible for overtime. 
 
Work Location: 

The home base for this position is Chinook Building located in Downtown Seattle, Chinook Building, 401 5th Ave., Suite 1000, Seattle.  This program is intended to provide direct services and will be working with clients in person. However, until is safe to return to the our locations, work will be done virtually. 

Employees will have access to shared workspaces at various King County facilities. Employees must reside in Washington state and within a reasonable distance to their King County worksite to respond to workplace reporting requirements.
 
Employees will be provided with a County issued laptop and must maintain a workspace with an internet connection (access may be supplemented in some situations) where they can reliably perform work and remain available and responsive during scheduled work hours. Please note that when an employee conducts work that is likely to bring them in contact with another individual, safety precautions are required, including the wearing of masks in some situations. King County is doing its part to reduce the spread of COVID-19 and remains committed to reducing our carbon footprint.
