Our Social Work & Care Coordination Department has an outstanding opportunity for a temp/hourly, day shift, SOCIAL WORK ASSISTANT 2. The individual in this position will assist inpatient teams with ensuring safe and coordinated discharges for patients of UWMC. This position works closely with Physicians, Nursing, Social Work, other members of the medical team and the patient to arrange follow-up appointments post-discharge, transmit the treatment plan to primary care physicians, skilled nursing facilities, home health agencies. and assist patients with transportation and other resources. This position serves as a member of the University of Washington Social Work and Care Coordination department.

This is a part-time, temporary/hourly (per diem) position to cover staff vacations. Schedule will vary, but will be expected to work some Saturday/Sunday shifts. This position has minimal direct patient contact.

This position serves as a member of the University of Washington Social Work and Care Coordination department, primarily assisting the Social Workers with the coordination of the treatment and discharge planning process for patients.

RESPONSIBILITIES:
  • Assist in arranging for Skilled Nursing and other post acute care placements post-discharge, based on information obtained from clinical staff                 
  • Assist in arranging home services, such as home health, home infusion, wound vacs,or in-home caregivers
  • Assist in arranging practical needs and emergency assistance, such as transportation, parking, food or local lodging. Demonstrate problem solving skills in working with resources outside the medical center
  • Assist the social worker and the patient/family in locating non-medical housing (respite, shelter)
  • Research and update community resource information as needed                               
  • Assist patients and families in accessing educational resources relating to the medical and psychosocial impacts of their illness
  • Participate in the training of Social Work Students and new employees
  • Participate in practicum instruction by developing training manuals, online materials
  • Document interventions in electronic medical record

  • REQUIREMENTS:
    Bachelor of Social Work or a Bachelor's Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent.
    OR
    an Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent.

    Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration. 

    DESIRED:
    Ideal candidates have experience working in a medical setting, demonstrate strong multi-tasking and organizational skills, oral and written communication, and computer proficiency. Must work well with a variety of persons and teams and possess excellent customer service skills.
