As the Regional Community Health Manager this position will oversee the implementation of an enhanced community health improvement strategy that promotes health equity and wellness within the East Market Region.  This entails improving program and reporting accountability, increasing effective communication between community outreach caregivers within the region, elevating the value of community benefit programming and identifying opportunities to leverage aligned clinical and community health initiatives. This position will work closely with UH Government & Community Relations (GCR) to execute the strategy.

This position provides management to, and reviews the work of, community outreach managers to accomplish community health implementation plans and results on a day-to day basis.  The majority of interactions will be with UH Government & Community Relations leads, UH medical center leads, community outreach managers and external stakeholders to exchange information, plan, executive and evaluate program performance.

Essential Duties:

•             Oversee UH East Region medical centers’ engagement with community partners within the respective counties to develop and implement community health strategies according to federal and state community benefit requirements.  More specifically, to ensure that strategies are effectively implemented at the local level and are done through the lens of health equity. 

•             Work with a data analyst to evaluate progress, analyze return on investment and measure impact within 5 community health focus areas.

•             Oversee the completion of quarterly Community Benefit Scorecard and Community Benefit Inventory of Social Accountability (CBISA) reporting to monitor the level and type of community benefit investment.

•             Rebrand and/or develop hyper local wellness and youth career programs in conjunction with Government & Community Relations and Corporate Communications.  This will entail:

-              Facilitating alignment with other departments and initiatives within the system to leverage resources and bolster our effort

-              Increasing access to traditional employee- or patient-only programs, for community members

-              Working with Government & Community Relations to implement UH signature programs.

•             Work with community outreach managers in the East region to achieve identified performance outcomes in their respective community health implementation strategies

•             5 hospitals in 4 counties (Ashtabula, Geauga, Lake and Portage).

•             Participate in local, regional and national summits and associations to enhance professional development, establish critical relationships and elevate UH’s work in the public domain.

•             Collect stories and curate content for community health-specific newsletters, UH community benefit report and press coverage.

•             Lead regional huddles to proactively identify synergies between clinical and community health goals and objectives.

•             Co-lead Community Health Leadership Council meetings.

•             Participate on relevant internal committees such as the Committee Benefit steering committee, community impact/sponsorship committee, COVID outreach, etc. to ensure alignment with East Region goals

•             Some travel required between medical centers, for stakeholder meetings and to attend conferences

Education

•             BS/BA degree required 5+ years in related field

•             Master’s degree in social science or public health related field preferred

Experience & Knowledge:

•             Minimum 5-years of experience in community health improvement role

•             Combined clinical and community health knowledge preferred

•             Existing relationships with key partners engaged in community health initiatives preferred

•             Competency regarding social determinants of health (SDOH), health equity and population health improvement 

•             Passionate about holistic wellbeing, inclusive of the social influencers of health

This role encounters Protected Health Information (PHI) as part of regular responsibilities. UH employees must abide by all requirements to safely and securely maintain PHI for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
