As a Social Worker III this role will specifically focus on the Complex Care Program within Population Health and will work closely with a Psychiatrist, Counselor and a Dietitian in a very collaborative role. There also is an opportunity to help to build up the program as it continues to grow.

Essential Duties:

  • Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
  • Communicates/collaborates with the work team using a multidisciplinary approach.
  • Provides linkage to community resources to facilitate optimal care.
  • Provides age specific care to patients/families in all age groups.
  • Develops and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs
  • Maintains a current working knowledge of UH network services and promotes utilization of such services.
  • Provides guidance and serves as consultant to staff and other disciplines regarding   complex psychosocial and/or discharge planning issues/barriers.
  • Assists with orientation/education of staff.
  • Assists with Process Improvement/Quality Assurance initiative.
  • Participates in process of staff recruitment, interviewing, recommendations.
  • Serves as liaison for community agencies
  • Provides clinical supervision to LSW staff and/ or social work interns after achieving LISW-S  (supervisory) status.

Education/Expertise:

  • Master Degree in Social

Required Credentials, License and/or Certifications:

  • LSW (Licensed Social Worker) - Social Worker II requirement
  • LISW (Licensed Independent Social Worker) or LISW-S (supervisory status) - Social Worker III requirement

Experience & Knowledge:

  • Hospital / Heath Agency experience preferred.
  • Preferably 2 years employment post graduate degree and 3000 hours of supervision by  an LISW-S
