  • The Post Acute Care Ambassador will act as a liaison between the University Hospitals acute care settings and the post-acute settings.
  • This will be a collaborative effort to support the next site of care decision process for patients safe transitions of care.
  • Participates in comprehensive evaluation of patients to ensure appropriate next site of care at discharge from the acute care, rehab, and skilled nursing settings.
  • Acts as a resource and provides education to referral sources regarding home care requirements and regulations.
  • Develops and maintains positive, professional relationships with entity partners at all levels.
  • Meets productivity metrics for obtaining non-system referrals.
  • Participates in entity specific care transitions meetings and committees with a focus on improving patient outcomes, reducing readmission rates, and reducing overall system costs.
  • Conducts monthly on-site visits to assign entities to enhance referral source relationships.
  • Receives and reconciles patient throughput issues to ensure timely service recovery and an improved patient experience of care.
  • Full time hours-day shift 8:00 AM to 5:00 PM

  • ADN required, RN - Registered Nurse, BSN Preferred. 
  • CPR certification within a month of hire.
  • Valid driver's license/car insurance coverage.
  • 3 yrs home care exp.
