POSITION SUMMARY: Assumes responsibility and accountability for an assigned team of residents; provides direct supervision and oversight of C.N.A staff . Assesses, plans, implements and reassesses nursing care according to Pine Run Policies and Procedures and the Nurse Practice Act, by assuming responsibility for all activities on an assigned team of residents. Performs all duties in a professional manner serving as a role model for staff.            

ESSENTIAL RESPONSIBILITIES:

Assures Quality of Care      

  1. Ensures quality in the delivery of resident care.
  2. Monitors and ensures that the delivery of resident care provided is safe and appropriate by actively supervising nursing staff assigned to team.
  3. Responsible for making daily resident rounds to assess residents’ physical and emotional status, uses sound nursing judgment to intervene and modify care. Maintains accurate and current knowledge of residents' status, communicates with Physicians, CRNP any acute changes using SBAR assessment; makes suggestions to help reduce hospital readmissions.
  4. Reviews 24-hr. shift report for appropriate and accurate follow-through.
  5. Administers medications and treatments accurately and safely per physician orders. Ensures that medication supplies are available for oncoming shift. Completes medication documentation accurately and thoroughly.
  6. Provides phlebotomy services in a safe manner using appropriate technique. Ensures that intravenous lines are properly maintained and infusion is per MD orders. Draws blood from PICC lines. (RN only)
  7. Works collaboratively with hospice services as appropriate. Provides end of life care with emphasis on resident comfort and family support. Pronounces resident as deceased (RN’s only) and ensures that required documentation is completed accurately and in a timely manner.
  8. Completes required documentation in an accurate and timely manner. This includes but is not limited to, admission assessments, physician orders, progress notes, discharge papers, incident and accident reports and 24 hour reports.
  9. Actively participates in preparing residents for discharge by supporting the plan and completing necessary resident/family education, training and documentation.
  10. Develops, implements and revises as necessary an individualized plan of care for each resident; attends care conference as assigned.
  11. Monitors meal and fluid intake and facilitates the resolution of nutritional problems.
  12. Provides quality care, in a timely manner, able to complete assigned work in allotted shift, minimizing overtime. Provides direct oversight and supervision of C.N.A staff to ensure assigned work is completed in allotted shift, minimizing overtime.
  13. Communicates significant occurrences to DON/Nurse Manager.
  14. Addresses resident/family requests and needs and delegates as appropriate.
  15. Ensures that C.N.A associate concerns are addressed in a timely manner, involve Nurse Administration as appropriate.
  16. Performs chart reviews of medications, treatments, as assigned.

Supports Resident Care Staff        

  1. Provides atmosphere where staff can perform their duties competently.
  2. Ensure a safe working environment is maintained.
  3. Participates with a schedule that allows staff to attend mandatory in services on an as needed basis.
  4. Participates in orientation of new staff.

Maintains Financial Objectives     

  1. Monitors unscheduled overtime, takes daily meal breaks and ensures the same for C.N.A staff members.
  2. Provides recommendations for work and cost efficiency
  3. Supports’ adjusted staffing ratios according to current census and acuity.

  1. Stays current with standards of practice related to medication administration, gerontological and medical nursing and infection control issues. Is self- directed and responsible for ongoing professional development.

  3. Ensures staff compliance with Pine Run's EXPOSURE PREVENTION PLAN and assists in monitoring staff compliance with Universal Precautions.
  4. Adheres to Pine Run Policies and Procedures and Fire/Disaster Plan, as well as federal, state, JCAHO, and OSHA regulations.
  5. Willing to function as subordinate staff. (All RN, LPN. and CNA staff.)
  6. Attends mandatory educational programs.
  7. Communicates and problem solves with DON, Nurse Manager and other Department Heads.
  8. Participates in performance appraisals of N.A staff.
  9. Demonstrates ability to be flexible and to adapt to change

 OTHER RESPONSIBLITIES

  1. Orients new licensed staff to residents, duties and responsibilities.
  2. Provides direct resident care (bathing, dressing, toileting, grooming, feeding) as
  3. Is knowledgeable about federal, state, OSHA and other regulatory standards and participates in surveys.
  4. Attends regularly scheduled staff meetings and other meetings requested by Administration
  5. Attends mandatory education sessions. Recognizes educational needs for self and staff and discusses with Nursing Administration.
  6. Utilizes electronic devices such as the computer, faxes, etc. in a manner that is cost and time effective. 
  7. Ensures the medical devices are in proper working order and notifies maintenance and central supply when items are in need of repair.
  8. Demonstrates understanding and complies with Pine Run's EXPOSURE PREVENTION PLAN for occupational exposure of blood borne pathogens and infection control practices.
  9. Has current, working, knowledge of, and adheres to, policies and procedures of Pine Run Health Center, including fire/disaster plan.
  10. Supports the Mission, Values and Vision of Pine Run Community.
  11. Is knowledgeable of resident rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all residents in a safe, secure environment.
  12. Maintain confidentiality of all data, including resident, employee and operations data.
  13. Complies with current law and policy to provide a work environment free from sexual harassment and all illegal discriminatory behavior.
  14. Completes duties with minimal supervisory direction. Utilizes appropriate judgment and sound decision making.
  15. Promotes positive public relations with residents, family members and guests.
  16. Performs tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending on individual circumstances.
  17. Acts as house supervisor as needed.

JOB QUALIFICATIONS:

  1. Must pass required health screen and testing before employment begins and yearly thereafter, as applicable to the position.
  2. Must have an acceptable criminal history report as defined by the Older Adults Protective Services Act and its amendments.
  3. Graduate of an accredited school of nursing
  4. Current licensure by the State of Pennsylvania Board of Nurse Examiners – LPN or RN
  5. Demonstrates strong leadership qualities with the ability to coordinate and supervise others.
  6. Demonstrates professionalism in appearance and mannerisms.
  7. Maintains physical and emotional well-being.
  8. Possesses personal qualities such as kindness, sympathy, good judgment, integrity, loyalty and the ability to effectively relate and communicate with residents, families and staff.
  9. Interacts and communicates with residents, families and staff.
  10. Reads, writes and speaks the English language.
  11. Possesses strong decision making skills.
  12. Current CPR certification.
  13. Current intravenous therapy certification.
