SUMMARY

The PRN LPN at Gateway will provide nursing contact for children and adolescents ages 12-19 with mental health diagnosis and co-occurring diagnosis in the residential setting.  Nurse will assess new clients for vitals, determine their height, weight, blood pressure and discuss allergies, side effects of psychotropic medications, and effectiveness of medications being prescribed.  Nurse will administer medication and intramuscular injections.  This position will also provide ongoing communication to the full-time RN in relation to the client's needs.  Nurse will assist in care coordination between the doctor, pharmacy, clinician and parent and/or guardian regarding medication needs/refills.  Nurse will provide case management services to assist individuals in gaining access to needed medical, social, educational, and other services; provides rehabilitation and support for client population; serves as a clinical specialist for medication management issues; psychiatric monitoring; provides information, education, and referral, agency linkage, advocacy, and follow-up services, and coordination of care; utilizes center-wide and community resources as needed for client benefit; embraces the key values of case management: empowerment, normalization, rehabilitation, and continuity of care.  Nurse will administer all prn meds and package meds for client passes.  The nurse will carry out doctor orders, schedule, transport and document all EPSDT and dental according to policy, keep accurate Medication Administration Records and Nursing notes and logs. This nursing position will do individual client education on, but not limited to, hygiene, medication side effects and benefits, psychiatric symptoms, health concerns.  Also, this nurse will be an active team member with residential staff and actively engage parents in medical and psychiatric issues. Transporting clients in a personal vehicle or Center vehicle is an essential function of this position. F endorsement required.

This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment.

REQUIREMENTS

Education/Knowledge: LPN. Preferably with course work or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and intellectual and developmental disabilities. LPN certification required.

Experience: Experience in psychiatric nursing preferred.  Experience working with adolescents is also preferred.  Computer experience is helpful. 

Applicants should possess the ability to relate to individuals and families of varied ethnic and cultural backgrounds, ages, educational levels, and economic circumstances with respect and dignity.  Excellent verbal, written, and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as maintaining proper documentation.  Clinical staff members must possess reliable transportation, the ability to acquire an F endorsement on driver's license, and transport clients.

Physical: Exposure to biological hazards. Hearing of normal and soft tones as well as close eye work. Valid driver's license. Lifting up to 50 lbs. Pushing/pulling up to 250 lbs. Frequent sitting, standing, walking, bending, stooping, and reaching. To ensure the safety of the clients and staff, this position requires certification in and adequate implementation of verbal and physical de-escalation techniques (Handle with Care) that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking.
