Coordination of care and access of specialty and general medicine care and screening.Community resource referral, patient education, and navigation, entry point in to the assigned UH center. Actively participates in the process for operational and quality improvement. Involved in learning and self improvement. Provides leadership through communication, education and management. Provides advocacy for the patient and family. Tracks interventions and outcomes of patients and families who access the Center. Bachelor Degree in Nursing, BSN 5 plus years of experience RN - Registered Nurse Experience with multiple healthcare data sources including EMR applications, registration and scheduling systems, and medical claims systems.
