Join our team of competent and compassionate professionals as an Outreach Coordinator in the PACT (Program for Assertive Community Treatment) program at Behavioral Health Network!

 

**NOTICE**

In accordance with the BHN Mandatory COVID vaccine policy - all staff, interns, temps, residents and contractors are required to be fully vaccinated no later than day one of employment. Medical or religious exemptions will be offered and must be approved prior to starting work.

Your experience, knowledge and great skills will support the PACT program with the following:

  • Providing direct outreach services to individuals in the community.
  • Supporting the growth, independence, and safety of individuals in all settings.
  • Monitoring individuals’ overall health and well-being in their daily living and encouraging independence, competency and decision-making.
  • Assisting with all aspects of medication treatment.
  • Implementing strategies to help staff meet treatment plan goals of their caseloads.
  • Providing crisis intervention and emergencies.
  • Planning and providing opportunities for community activities.
  • Maintaining and fostering social support networks and relationships.
  • Completing and maintaining progress notes, health, and medication-related documents including initial assessments, treatment plans & reviews, and financial management plans.
  • Supervising staff and managing their schedules in addition to participating in supervisory on-call rotation.
  • Overseeing the completion of rep-payee responsibilities and client budgeting.

The PACT program provides clinical support to adults in the community.  PACT is a highly integrated outreach program that helps adults meet their goals and reduce hospitalizations.  The PACT team is forensic –focused and is designed to reduce arrests and incarcerations and also operates a 24/7 group residence for five individuals.

Job Requirements:

What are we looking for in an Outreach Coordinator?

  • Commitment to empowerment and growth, human rights, and community integration of individuals served (required).
  • At least six months of experience in human services (preferred).
  • Previous supervisory experience (preferred).
  • MAP certification (preferred, but to be obtained within introductory period of employment).
  • Bachelor’s Degree (required).
  • Valid driver's license and a reliable car available for working hours (required).
