The telephone triage nurse is a member of the multidisciplinary health care team and is responsible for conducting an accurate and reliable nursing assessment, disposition and documentation of health related issues, collaborating with providers to ensure patient safety and achieve the most optimal health care possible that is uniquely tailored for each patient as demonstrated by the following. Completes focused assessment of patient without the advantage of a face to face encounter with patient or caregiver. Collects sufficient information related to the presenting problem including medical history, pattern or severity of symptoms, current medications and any interventions that the patient may have already implemented at home. Demonstrates the ability to assess and utilizes critical thinking to determine the urgency or acuity of the situation and takes appropriate and immediate action. Effectively uses and applies information technology to give and receive information. Documents the assessment, plan and outcome of telephone interaction timely in the medical record. Provides and documents patient education. Responds to patient telephone calls in a timely manner. Conducts follow-up phone calls. Manages multiple priorities in an effective and organized manner while maintaining exceptional service to patient, caregiver, providers and care teams. Maintains composure when dealing with difficult situations and responds professionally. Accurately and effectively gathers quality and performance improvement data under the direction of the nurse manager.

Thorough knowledge of nursing process and practice. 
Demonstrates a professional demeanor when responding to patients. 
 A.D.N., diploma in nursing or BSN 
Registered Nurse with current license to practice in the State of Ohio
1+ years of RN experience and oncology experience highly preferred
