(LCSW-R) for Primary Care Office

Primary Care office in Albany NY seeking LCSW-R to provide integration of behavioral health services in the primary care setting, a newer model of care being promoted by CMS and insurance carriers. We are a privately owned practice with six providers in a convenient location. Candidate will promote integration of behavior through identification of patient-specific interventions and new programs, will serve as an "internal consultant" to providers and staff, will support patients through in-office counseling based on the integrated model and/or referrals to appropriate specialists and community services. The overall goal of this model is that of maintaining and enhancing the mental, emotional, behavioral and social functioning and well-being of patients of the practice. Minimum two years' experience in medical social work in an ambulatory setting with at-risk patients, knowledge of available community resources and the ability to evaluate resources and make appropriate referrals and knowledge of health-related financial assistance, including Medicare and Medicaid required.
