JOB SUMMARY: Assists in record retrieval and filing. Enters confidential patient information in the hospital information system.

QUALIFICATIONS:

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Analyzes all records daily for specified elements of completeness.
  • Reviews a random sample of medical records monthly for quality monitors.
  • Retrieval of documentation from various departments throughout the hospital daily at assigned intervals.
  • Monitoring of H&P and Operative reports as back up.
  • Quality analysis of documents scanned outside of the department.
  • Scanning and indexing of documentation when applicable.
  • Assists in assignment of volunteer responsibilities.
  • Other duties as assigned.

EXPECTED BEHAVIORS:

  • Verifies each page has correct patient name, medical record number and account number.
  • Remove and appropriately index any pages that do not belong to the patient.
  • Accounts for 100% of records by verifying received records against daily logs
  • Reviews records for requirements listed in the “Medical Staff Rules and Regs” and/or “Medical Record Completion Policy”.
  • Attaches colored tabs where signatures or additional documentation is needed when applicable.
  • Retrieves documentation from the appropriate location.
  • Performs quality checks on patient charts according to specified indicators.
  • Reviews at least 50 charts per month of inpatient and 50 charts a month of outpatient.
  • Daily analysis of inpatient and outpatient records.
  • Prints surgery schedule from surgery and checks to see if all surgeries have an operative record within 24 hrs., if applicable.
  • Assigns deficiencies as appropriate.
  • Assists customers and patients in release of information requests.
  • Assists physicians with questions and miscellaneous issues.
  • Answers the telephone within two rings with a “customer service attitude”.
  • Scan assigned documentation daily.
  • Merge duplicate medical record numbers within Meditech.
  • Log release of information forms daily.
  • Assign deficiencies as needed.
  • Safeguards the privacy of all medical information.
  • Demonstrates ability to embrace and adapt to change. Regularly initiates ideas to improve the efficiency and processes of the department in a positive and productive manner by attending at least 90% of the annual department meetings.
  • Displays and maintains a “customer service attitude” which promotes good guest relations to patients, visitors, practitioners, and co-workers.
  • Aptitude for detail and accuracy.
  • Organized and neat.
  • Evidence of good grooming and professional conduct.
  • Initiative.
  • Ability to work unsupervised.
 
Position Requirements

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • High School graduate or equivalent. Previous healthcare experience preferred.
  • Knowledge of alphabetic and numeric filing systems.
  • English grammar.
  • Experience in computer operations.
  • Types accurately at least 30 WPM. 6. Mandatory Continuing Education: Customer Service, Fire and Safety, Corporate Compliance (including Confidentiality), Infection Control, and education required by regulatory, accreditation bodies, scope of practice, and/or Hancock Regional Hospital.
