The UH Population Health Provider Value Network Strategist are a multi-disciplinary dedicated to  transformation focused on Value, Network Integrity and Provider/staff engagement who work closely with primary care clinicians, specialists, and other system leadership to transform and advance value-based care.  The role works seamlessly within and as a member of the Provider Value Network Team (PVN Team)

Key responsibilities include:

  • Will focus on the three pillars of: 1) value, 2) provider engagement, 3) network integrity
  • Will be heavily focused on the employed UHPS Practices, but will also work collaboratively with other members of the PVN Team for sharing of best practices, tactics and cross coverage for providers in the QCN and independent physicians
  • PVN Teams will be strategically organized by geography, with accountability for Value, Network Integrity and Engagement of providers in each market. 
  • The PVN will be a multidisciplinary team that will develop strategies and tactics, informed by and outcomes measured by data
  • Develop, iterate, execute, and update value, network integrity and engagement scorecards/value matrixes and provider playbooks
  • Participate in provider and practice engagement through in-person, virtual, and written communications
  • Optimize wellbeing through facilitating practices to improve quality gap closure rates and annual wellness visits rates
  • Partners closely with population health and other value-based external organizations to drive results and network development
  • Utilizes Salesforce platform to track activities and correlate focused work activity to outcomes
  • Self-starter and self-directed change agent that is passionate and dedicated to practice improvement and transformation while advancing healthcare in alignment with the quadruple aim to support improvements of better health, better care, provider well-being, and improved value.
  • Builds, maintains, and facilitates relationships with practices and other teams that support practices and population health
  • Builds, maintains, and facilitates relationships with appropriate hospital leadership and Institute/service line leadership
  • Responsible for knowing and understanding practice dynamics, practice design, practice relationships of various provider practices to become subject matter expert on the practice from a population health perspective
  • Coach and educator to practices as well as other teams that support practices in facilitating population health initiatives and quality improvement leveraging quality improvement tools
  • Monitors, summarizes,  and analyzes data/outcomes, develops summary reports designed to trend outcomes, and assist with ongoing evaluation, monitoring and assessment of practice specific outcomes
  • Leverages various data platforms to provide practice centric updates and analyzes data and trends to educate practices on current outcomes
  • Translates data into easily understandable and actionable information that can be used to collaborate with practices on specific interventions to optimize outcomes and meet quality metrics, Population Health goals, and other KPIs  
  • Develops presentations, education, collaterals, and tools to facilitate practice advancement to optimize practice performance in collaboration with UH Population Health team members
  • Autonomous, agile, and versatile in balancing multiple initiatives, facilitation at multiple practices, educational interventions, providing scorecards, while meeting appropriate timelines and deliverables.
  • Supports  practice sites in identifying and setting meaningful S.M.A.R.T. goals  and relevant data-driven goals and improvement measures to optimize outcomes
  • Facilitates and supports practices in developing workflows, practice centric interventions, and quality improvement by leveraging quality improvement methodologies such as PDSAs and measurements to advance practice transformation
  • Embraces multi-functional worker concept by performing tasks that are needed for a start-up company before other staff members are hired
  • Leverages expertise in practices to collaborate and design interventions and initiatives to advance primary care. Uses knowledge of data analysis and statistical processes to translate data into actionable information that teams can use to make decisions affecting patient outcomes.
  • Collaborates with the Manager, Quality & Clinical Transformation, Operations team and other Population Health team members in optimizing and supporting practice interventions, collaterals, and education as well as miscellaneous activities to improve and support population health, QCN, and UH initiatives.

Education

  • Bachelor’s Degree Required 
  • Master’s Degree Preferred but not necessary

Experience & Knowledge:

  • Quality Improvement & Process Improvement skills preferred
  • Healthcare / Quality /Public Health/ Change Management/Population Health/Practice Support preferred

Special Skills & Equipment Knowledge:

  • Proficient in Microsoft Office suite required. Excellent time management and interpersonal skills. Ability to manage multiple projects and prioritize.  Able to work independently.  Familiar with LEAN, Gantt charts, and other QI/change management methodologies.
