Job Summary: In this position the Social Worker will provide comprehensive psychosocial care and support to the diverse outpatient population of the Child and Adolescent Pediatric Clinic. This position works collaboratively within the multidisciplinary team and meets requirements for PCMH. The Social Worker provides comprehensive psycho-social management and support to at risk patients and families.  Duties include providing psychosocial consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patients and families, coordinating discharge planning activities for patients requiring services or facility placement after discharge; documenting all interventions. Participate in departmental meetings, in-services, education, and quality improvement activities; and performing other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.  Minimum Qualifications:

M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. *New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit.  Must obtain L.M.S.W. within 12 months of receipt of limited permit.

Preferred Qualifications: Previous Ambulatory experience desired with children & adolescents.  Knowledge of dialectical behavioral therapy helpful. Work Days:
