Summary:
The registered nurse is a member of the health care team. Is responsible for providing quality nursing care utilizing standards of care. Will uphold the Philosophy and Objectives of Nursing Service. Is a resource person for other members of the health care team. Will perform other duties as designated by the director. Responsible for patient care. Will utilize effective communication between personnel, physicians, other hospital departments, patients and families. Administers I.V. medication, hyperalimentation, blood and blood components according to hospital policy.

1.Previous Experience Required:

Three years clinical nursing experience preferred of which at least one year was in a direct patient care area.

Utilization Management/Case Management experience preferred.

Previous rehab experience preferred.

Previous Psych experience preferred.

2.Specialized or Technical EducationRequired:

Registered Nurse currently licensed to practice in the State of Louisiana.

Knowledge of Prospective Payment System, HMO’s and PPO’s preferred.

Surgery/ICU/ER experience helpful.

Knowledge of ICD-9-CM coding preferred.

3.Manual or Physical Skill Required:

Basic computer skills preferred.

Responsibilities:
I. Insure the maintenance of quality patient care.A. Utilize nursing process (assess, plan, implement and evaluate) for each patient on an individual basis.B. Makes and demonstrates effective assessment skills, to provide medical care and correlative attainable goals.C. Responsible for care plans.1. Review and carry out plan of care based on assessment and established plan of care, delegate patient care after identifying priorities of patient needs and problems.2. Utilizes patient, family, and other disciplines involved in patient care, to establish patient/family goals and plan of care.3. Effectively communicates plan of care with all staff members.D. Evaluate and document effectively the patients’ progress and participation in the rehabilitation program providing further goal setting for patient and family.E. Actively participates in weekly trans-disciplinary summaries through documentation. Verbally during staffing evaluates plan of care and establishes goals. Also to evaluate if discharge planning is progressing and to revise treatment plan as required.F. Assist with chart audits.H. Administer medicationsI. Responsible for patient care.J. Transcribes physician orders according to policy/procedure.K. Utilizes equipment safely.II. Administer blood, blood components, hyper-alimentation and chemotherapy.A. Adhere to hospital policy in regard to blood and blood components.B. Adhere to hospital policy for administering hyper-alimentation.III. Care of the patient in the Transitional Living ProgramA. Evaluates with transdisciplinary team the patient in the transitional living program.1. Assist family in setting reachable goals for them selves and the patient.2. Evaluate patient/family's ability to function with 24hr. nursing care.3. Refer to rehabilitation's list of patients’ functions, document problems and concerns.4. Demonstrate proper techniques, but allow caregiver to complete task.IV. Follows North Oaks Health System’s Compliance Program and Federal and State Regulatory guidelines.V. Is able to demonstrate competency in age specific criteria as applicable to job scope.SOCIAL & EDUCATIONAL SKILLSDemonstrate effective communication skills with patients, visitors, families and staff members.Assist in the education of patients, family, present staff and new staff members.PERSONAL DEVELOPMENT AND UNIT PARTICIPATIONAttend appropriate in-service training to learn new skills and enhance existing skills.Participates in promotional activities and marketing of the rehabilitation unit.Become familiar with and maintain familiarity with JCAHO, OSHA, CMS and CARF standards.Participate in the Quality Improvement Program established by hospital.Maintain competency in rehabilitation skills and techniques.Personal development suggestions to include but not to limit to:Earning credit toward a baccalaureate or masters degree program.Certification by Rehabilitation nursing association.Volunteer for community service for the benefit of the hospital.Certification in ACLS.Attend rehabilitation courses offered.FIM CertificationIn addition to those outlined in the general RN job , the NORH Nursing Administration RN duties include the following:1. Responsible for the following a. Keeps abreast of all current and proposed rules, regulations, policies and procedures related to the CMR inpatient rehabilitation program which requires daily collaboration between the Intake Nurse and the CMR Outcomes Analyst.b. Maintains knowledge of screening criteria used in the admission process and is able to apply them objectively.c. Maintains knowledge of documentation necessary to submit for insurance approval for the CMR program.d. Maintains the referral list for the CMR program through electronic work queues.e. Functions as a liaison between the NORH/CMR program Medical Director, referral sources, patients and family members.f. Prioritizes the CMR program referral list daily according to the psychosocial, medical, economic/financial, and discharge planning needs of our referred patients.g. Comprehensively assesses and documents on the pre-admission screening document on all patients referred to the CMR program.h. Communicates any patient needs to the appropriate professionals for follow-up.i. Gathers and records necessary demographic and marketing data on all admission calls to the CMR program. Presents findings to pre-admission staffing team for recommendation on placement of patient.j. Communicates with and presents a comprehensive assessment along with other requested documents to insurance companies for admission acceptance to the CMR program.k. Submits any necessary supporting documentation when requested by insurance companies on denial claims for admission to the CMR program.l. Collaborates with the interdisciplinary team, patient, caregivers and other team members to develop a transition plan for admission appropriate to the patient’s needs.m. Presents written insurance disclosure information to all patients prior to admission to the CMR program.n. Presents and extensively reviews program care plan expectations with patients and caregivers. Maintains ongoing communication until admission to CMR is complete.o. Communicates appropriate alternate recommendations to Social Workers, Case Managers, Physicians, patients and caregivers if patient is unable to meet the for admission to the CMR program.p. Refers cases which do not meet criteria for admission to the Nursing Director and/or Physician/Nurse Practitioner and assists in coordinating the review process for cases.q. Resource person for documentation, placement, reimbursement issues and referral sources.r. Conducts marketing calls to referral sources as directed.s. Is able to demonstrate competency in age specific criteria as applicable to job scope.t. Fosters positive internal and external customer relations.u. Follows North Oaks Health System’s Compliance Programs and Federal and State Regulatory guidelines.
