Social Worker II Geriatric Case Management- Social Work

Location: MetroHealth Medical Center

Shift: Full-Time, Days - 80-Hours Bi-Weekly: 7:30a-4p

Summary: 

Participates in the diagnostic evaluation of patients. Utilizes a thorough knowledge of social science and social work skills to identify the patient(s) needs, and delineate the appropriate social work treatment(s)/intervention(s) to children through adult patient populations. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.

 

 

Responsibilities:

 Contributes to patient safety by:

a. Providing a safe environment for patients.

b. Reporting safety concerns, errors and near misses.

c. Participating in system improvement initiatives to prevent errors.

d. Providing evidence-based care as applicable to the role.

1. Provides clinical Social Work services by:

a. Providing knowledge of the impact of acute/chronic illnesses and disabilities to patient(s) and family members.

b. Obtaining the history of a patient(s) social situation.

c. Providing short or long-term counseling to individuals/groups/family members to promote the reasonable acceptance of medical conditions, and aiding in the adjustment of all parties to the patient(s) immediate medical situation; working toward the optimization of subsequent social functioning.

d. Collaborating with other professional disciplines within The MetroHealth System and the external community to facilitate communication regarding the patient(s) condition/treatment plan, and ensuring full cooperation between all parties.

e. Functioning as an advocate for patient(s) within The MetroHealth System and the external community to ensure the protection of the patient(s) personal rights, and the optimal use of societal resources.

2. Assists patient(s) and family members with preparing a care plan that will maintain patient(s) at the maximum level of functioning upon return to the community.

3. Makes home visits as needed.

4. Encourages patients to comply with medical recommendations (i.e. clinic appointments, treatment instructions, etc.).

5. Participates in Social Work Department staff meetings and staff development programs.

6. Fulfills all accountability requirements of The MetroHealth System, the Social Work Department, and all outside regulatory bodies.

7. Keeps the immediate supervisor informed of developments in all cases and problems.

8. Performs other functionally related duties as assigned.

Note: While this position has no direct supervisory responsibilities, the employee will monitor the activities of the non-employee student interns.

Required:
 Master’s Degree in Social Work from an institution accredited by the Council on Social Work Education. Must be a current Licensed Social Worker (LSW) for the State of Ohio, or possess a temporary Social Work licensure for the State of Ohio. 

Preferred:
NonePhysical Requirements: May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. May need to lift up to twenty-five (25) pounds on occasion. Ability to communicate in face-to-face, phone, email, and other communications. Ability to see computer monitor and departmental documents.
