Job Description

Job Summary
Ensure timely and effective delivery of health and wellness programs and services through the personal delivery and/or coordination of such services. Provide education, support and navigation assistance related to health/wellness to individuals and their families during personal interactions including home visits, telephone calls, etc. Educates individuals and their families about wellness, chronic disease prevention and self-management. Develops and maintains community and provider relationships, along with providing valuable documentation of experiences to evaluate programs and services offered. Cultural competency and shared-life experiences with client population is preferred. Must have reliable transportation and be able to drive to and from appointments.

Essential Functions
• Communicates effectively with individuals and families to assure satisfaction and continued relationships. Assists with issue resolution in a manner that meets or exceeds customer expectations.
• Develop and maintain community-based organization and provider office relationships with continued visibility, contact and collaboration on department goals.
• Assist with Health screening activities as needed.
• Assists with distribution of promotions, including newsletter, promotional flyers, educational mail-outs and e-mail updates.
• Actively participates and collaborates with Medicaid Quality team in achievement of department and organizational goals and objectives. Serves as Priority Health Outreach Representative for designated events, internal/external committees, service organizations and functions as appropriate.
• Communicates and collaborates with the clients and their families, program staff, and other professionals involved in the care of clients to coordinate services and to facilitate a supportive relationship between the client and their health care provider(s). Serves as a resource to program staff about situational and cultural factors impacting the client, family, and environment. Teaches clients and families about wellness, disease prevention, and self-management.
• Documents services including assessments, plans, and periodic evaluations of goals, education, and other pertinent program data. Documents and utilizes trends and changes in the health of the community to provide input into program development and evaluate effectiveness of services provided and to develop a proactive plan for future needs.

Qualifications
• Required High School Diploma or equivalent
• Preferred Associate's Degree health related field
• Preferred Knowledge and use of community resources; social, health and public agencies and services.
• Required Drivers License - Auto Insurance
• Community Health Worker Certificate preferred
