UW Medical Center - Northwest has an outstanding opportunity for a full time, day shift REGISTERED NURSE to join our Primary Care Team in North Seattle. The Registered Nurse will play a critical role providing ongoing case management and coordination through UW Medicine’s continuum of care.  

RESPONSIBILITIES: 
As a Registered Nurse, you will follow evidence-based nursing practices, exercise independent judgement, and manage complex primary care health issues. You will collaborate with patients, their family, and other healthcare providers to develop and implement treatment plans. You will interact regularly with providers and support staff across the UW Medicine system, including pharmacies, hospital personnel, laboratories, and home care nurses. Additionally, you will provide assessments, planning, education and evaluation of the patient’s response to treatment, and will deliver services through scheduled in-person visits and via telehealth.

REQUIREMENTS: 
  • An active Registered Nurse license in the State of Washington
  •  
    LEGAL REQUIREMENTS: 
  • BLS Certification
  •  
    DESIRED: 
  • Bachelor’s degree in nursing (BSN) 
  • Ambulatory Nurse or Care Management Certification 
  • 3 years of experience in care management, home health care, or primary care/triage
