COORDINATED INTAKE & REFERRAL COORDINATOR (CONTRACTED)
FULL-TIME POSITION (WORKING REMOTELY)

Annual Pay: $32,960

QUALIFICATIONS & REQUIREMENTS

  1. BA/f3S/AS/AA in Human Services or related field in an accredited program.
  2. Computer skills with knowledge and experience in databases, Microsoft Office (Excel, Word, Outlook) and internet are required.
  3. Manage a caseload and maintain performance measures, organize and prioritize work assignments; make decisions and independent judgments; determine the appropriate course of action in emergency or stressful situations; and recognize reportable events.
  4. Must communicate both verbally and in writing.
  5. Bilingual skills in English and Spanish preferred
  6. Must have transportation

POSITION SUMMARY

Under the direct supervision of the Connect Supervisor, the Intake Coordinator provides administrative services to pregnant women and infants that have completed a Prenatal or Infant Healthy Start Screen. referred by community partners, or Managed Care. These administrative services are provided on the telephone. Services include education and referral to agencies and resources that address concerns like maternal and child health, positive parenting, safe home environments, and access to other community resources. Must observe and maintain confidentiality of information

PRINCIPAL DUTIES

Client Contact:

  1. Initiate and complete initial client contact, within the specified program tirrieframes and guidelines, based on information provided on the prenatal screen, infant screen, SOBRA Medicaid, or referral.
  2. Determine program(s) eligibility based on the information received on the prenatal or infant risk screen or from the client during the intake interview. Take appropriate actions (e.g. either refer the case to the appropriate agency or close the case) based on these findings.
  3. If client declines services, and case is closed, provide the client with materials to educate and improve clients knowledge and understanding of available community resources and services that support the well­being of women during pregnancy and/or during child's infancy (e.g. WC, food banks, housing, mental health service, etc.). The information provided should be based on the risks identified through intake process.
  4. Follow appropriate telephone etiquette when answering incoming calls, leaving messages (e.g. identify yourself, the program, and speak clearly when leaving messages, etc.).
  5. Develop and maintain comprehensive understanding of maternal / child health risk factors identified during the interview process for pregnant women and infants.

Documentation:

  1. Generate necessary correspondence in compliance with program guidelines.
  2. Maintain daily case documentation in Well Family System.
  3. Develop and maintain working knowledge of program's Well Family System and State of Florida Healthy Start Standards and Guidelines.
  4. Prepare and submit appropriate administrative reports (e.g caseload reports, timesheets, mileage reports, etc.).

  3. Maintain the Healthy Start Coalition's quality service standards.

Additional Duties:

  1. Maintain training schedule
  2. Participate in supervision and team meetings, as requested.
  3. Abide by all Healthy Start Coalition policies and standards of conduct.
  4. Perform other duties as assigned.
  5. Bilingual skills in English and Spanish preferred
  6. Must have transportation
  7. Pass a level 2 background check drug screen
