Coordinator, Weight Management

 

SCOPE OF ROLE

Reporting to the Director, Primary Care Services, the Coordinator, Weight Management will create positive engagement opportunities of healthy lifestyle choices by providing support, education, goal setting, encouragement, and guidance to patients.  You will be relied on to participate in treatment plan development and provide assessment and treatment services to patients and their families to increase their well-being through awareness, motivation, and action to reduce, eliminate, or manage health issues affecting progress in the program.  You will be collaborating with all SH clinical service lines to achieve the goals of the program and its participants.

 

HOW YOU’LL SUCCEED ____________________________________________________________________

  • Provide outstanding comprehensive, evidenced-based nursing care with a focus on the patient service experience, sensitive to the morbidly obese.
  • Perform nursing procedures by promoting a whole-person approach.
  • Navigate patients through outpatient weight management services including but not limited to medical history taking, diagnostic testing, treatment and management of physical and behavioral health conditions, behavioral risk assessment, subsequent counseling and referral, preventative care and screening, encouraging treatment adherence.
  • Create a comfortable environment when escorting patients to exam room; interview patients and measure vital signs including but not limited to biometric measurements, body composition analysis, EKGs, and blood draws; documenting information in patient’s chart.
  • Encourage, monitor, and follow up on patient engagement and participation in all treatment plan activities.
  • Consult and collaborate with patients, clinical and behavioral health staff, and providers to correctly execute appropriate plan of care, a multidisciplinary approach.
  • Assist and encourage patients in wellness vision and goal planning.
  • Prepare, administer, manage, and provide education on medications and prescriptions ordered by prescribers.
  • Build the knowledge of your patients by understanding their comorbidities, challenges, what will make them successful.
  • Provide comprehensive (based on patient’s learning style) instructions on physician directives, treatment procedures, medications, nutrition, exercise, chemical dependency cessation, etc.
  • Provide ongoing assessment of the patient’s key family members’ needs and personal support systems.
  • Perform client specific advocacy and/or review of utilization of services.
  • Serve as a subject matter expert for patients when answering questions and navigating the patient through the Weight Management Program.
  • Stay up to date on the science behind the nutritional guidelines by researching quality medical studies/journal articles.
  • Facilitate group discussions with patients focused on nutrition, weight management, and medication education and support.
  • Conduct outstanding group experiences, educational activities and opportunities both onsite and in the community focusing on lifestyle, prevention, screenings, nutrition, physical activity, tobacco cessation, etc.
  • Create excellent experience when conducting health fairs and wellness clinics on a scheduled basis.
  • Respond to telephone and EPIC in basket messages, providing feedback and answers to patient/physician/pharmacy/stakeholder inquiries.
  • Ensure all related reports, labs, and information are filed and available in patient’s medical records prior to their appointment.
  • Maintain clear communication with co-workers, referents, grant/board requirements, and other designated liaison persons.
  • Collect and report aggregate and individual results on identified outcomes measured on an agreed upon cadence with the Weight Management Director. 
  • Participate in quality assurance and utilization review activities.
  • Willingness to incorporate nutritional guidelines and exercise into personal life preferred.
  • Comply with all agency policies and procedures.
  • Maintain all aspects of HIPAA, promoting patient confidentiality and right to privacy.
  • May be required to perform telehealth services as determined by Signature Health.
  • Adherence and completion of compliance training provided by Signature Health.
  • Responsible for and completes all productivity/documentation requirements.
  • Participate in all assigned staff meetings and staff development programs.
  • Demonstrate appropriate attendance and punctuality in adherence with Signature Health policies.
  • Other duties as assigned.

 

KNOWLEDGE & EXPERIENCE ______________________________________________________________

  • Completion of an approved program in registered nursing required.
  • Valid unencumbered RN license in the state of Ohio required.
  • 1+ years of weight management, nutrition or related experience required.
  • 2+ years of clinical nursing experience required and wellness program support preferred.
  • Bachelors of Science in Nursing (BSN) preferred.
  • American Heart Association (AHA) Basic Life Support (BLS) certification required.
  • Skilled in motivational interviewing technique preferred.
  • Proficient in operating mobile technology, managing a patient registry, Microsoft Office Suite, and Electronic Health Records.

WORKING CONDITIONS ___________________________________________________________________

  • Work is normally performed in a typical interior/office/clinical work environment.
  • While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.
  • Requires periods of sitting, standing, telephone, and computer work. 
  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.
  • Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.
  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
  • Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.
  • Possible exposure to blood borne pathogens while performing job duties.
  • Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.
  • Sufficient dexterity to operate a PC and other office equipment.
