Job Summary

We're recruiting a highly talented and motivated advanced practice provider to support our cardiac transplant and ventricular assist device program. This APP will coordinate with many other providers and professionals to provide top level care of our transplant and VAD patient population. This is a non-surgical position focusing on preoperative evaluation, postoperative management and ambulatory concerns for this specialized patient population.

  • Participate in daily rounds with the multidisciplinary team.
  • Daily assessment of preoperative and postoperative transplant and VAD patients.
  • Maintain communication between the multidisciplinary team and have a thorough knowledge of the VAD and transplant patients.
  • Provide continuation of care of the VAD and transplant patient throughout inpatient and outpatient settings.
  • Provide communication to the VAD and transplant patient family including participation in family care conferences.
  • Represent the cardiothoracic service at VAD/heart transplant and lung transplant selection committee meetings.
  • Provide primary coverage for the cardiothoracic surgery service during the evaluation and workup of the transplant and MCS patient population as well as care for the immediate and subsequent re-admission of post op patients.
  • Perform consults of VAD, heart transplant, lung transplant evaluations. Perform consults for other surgical concerns as it relates to VAD and transplant patients, including drive line infections.
  • Management of surgical and wound concerns in the VAD and transplant patients in the Heart and Lung Transplant Clinic.
  • Preparing patients for transplant in regard to the pre-transplant plan and checklist. Also maintaining a working knowledge of patients on the heart and transplant waitlist, including participation in weekly pre-heart transplant risks review meetings.
  • Participate in weekly VAD/Transplant MDT rounds and discharge planning meetings.
  • Maintain knowledge of post-transplant patients including weekly outpatient post heart transplant rounds.
  • Chronic driveline infection management including monthly review meeting and frequent clinic appointments.
  • Procedures: chest tube management and removal, wound care including complex drive line and wound vac management, surgical site evaluation and management, management of Impella and axillary IABP devices in pre-transplant patients
  • Familiar with the surgical anatomy and transplant procedures. Working knowledge of physiologic consequences of heart and lung transplantation and incorporate that understanding in the preop workup of patients as well as the post op management of patients
  • Working knowledge of MCS including ECMO, Impella, Heartmate II, III, Heartware and Centrimag technologies.
  • Detailed knowledge of UNOS (United Network of Organ Sharing) and how heart and lungs are placed in the United States.
  • Works with the multidisciplinary team to develop department and hospital policies regarding transplantation and VADs, including VAD and transplant quality and safety meeting

Under physician supervision, provides medical care to patients of varying type and acuity.

Essential Functions

  • Functions in the role of advanced practitioner to coordinate comprehensive care delivery. Writes orders/prescriptions for standard formulary medications, laboratory, radiographic studies and consultations, coordinating the scheduling of admissions, diagnostic evaluations, follow-up exams, laboratory tests, referrals to home care, social services, and other community agencies, etc., in order to ensure continuity of care and coordination of services.
  • Obtains histories and performs physical exams.
  • Assesses the patients/significant other, health care team, school and other community educational needs and develops, implements, evaluates and revises teaching strategies/educational programs related to area of specialty.
  • Acts as a patient advocate, crossing multidisciplinary lines to meet patient needs.
  • Communicates, collaborates, networks with and acts as a consultant to other members of the health care team within and outside Spectrum Health. Acts as a consult in area of expertise.
  • Performs and assists with designated procedures.
  • Documents in the medical record and dictates information as appropriate.
  • Provides input into the strategic planning, implementation and evaluation of service/program. Measures the outcomes of care and evaluate services across the continuum of inpatient to outpatient services. Makes recommendations to improve the quality of patient care provided and utilizes the scientific method of research clinical problems.

  • Basic Qualifications:

    Education - Graduate of an accredited Physician Assistant educational program

    Education - Nurse Practitioners who obtained their education and certification after 2000 must show evidence of completion of a master’s, post-master’s or doctorate from a Nurse Practitioner program that is accredited by the Commission on the Collegiate of Nursing Education or the National League for Nursing Accrediting Commission

    Experience - 3 years of experience typically gained through skills/knowledge/abilities in current, relevant clinical experience Previous experience functioning in a collaborative role
