Position is at Cleveland Medical Center, can be 4 - 10's and 50/50 work from home after 90 days.

UH Seidman Cancer Center SCC RN Patient Navigator provides high touch customer service to customers, while building strong relationships and helping them navigate through the healthcare system. Coordination of care and access of specialty medical hematology, oncology, surgical oncology and radiation oncology care. Community resource referral, patient education, and navigation at the entry point and across the continuum of care. Actively participates in the process for operational improvement and quality improvement. Involved in learning and self improvement. Provides leadership through communication, education and management. Provides advocacy for the patient and family. Initiates and maintains positive relationships with coworkers and physicians. Demonstrates commitment to the mission, vision and values of UH. Outstanding written and verbal communication skills. Comprehensive understanding of local medical systems and resources. Understands multiple computer systems.  Thorough knowledge of nursing process and practice. Fluent in English both verbally and written.ing, but not limited to UHCare Oncology EMR, Sorian Scheduling, EHealth, and PACS. Management and handling of patient records. Excellent time management skills and proactive approach to the needs of SCC customers.

Bachelor Degree in Nursing, BSN

3-5 years of experienceas a RN, previous oncology experience preferred
