Summary: 

Under the direction of the Nurse Manager or assigned Registered Nurse, provides various patient care duties for the rehabilitation patient. Assist with activities of daily living, directs patients in activities that support the rehabilitation plan, and documents in patient Electronic Medical Record (EMR). Delivers and assists with patient meal services, stocking supplies, patient transport, messenger functions and cleaning. Ensures compliance with all safety, sanitation and quality control requirements. Upholds the mission, vision, values, and customer service standards of The MetroHealth System

 

 

Responsibilities:

1 Provides constant patient companionship by:

a. Observing patients continually throughout shifts.

b. Encouraging memory techniques using the memory notebook, memory board, calendars and pictures.

c. Facilitating diverse activities to individual patients or a group of patients under the direction of a RN.

d. Using distraction, diversion and redirection techniques.


2. Provides general patient care support to the nursing unit by:

a. Assisting patients with grooming and all aspects of personal hygiene and/or assembling supplies, equipment, and prostheses for patient’s own use.

b. Reporting any unusual behaviors or conditions to the proper personnel; records pertinent data.

c. Lifting, turning, positioning and transferring patients (utilizing mechanical devices if needed).

d. Maintaining patient safety and comfort by: utilizing pressure-relieving devices, utilizing restraints/restraint alternatives, aiding in ROM exercises and applying/removing protective devices (i.e. helmets, braces etc.).

e. Distributing water, nourishment and meal trays and assisting patients to eat and drink as needed.

f. Emptying drainage container(s), measuring and recording known intake and output excluding intravenous intake.

g. Connecting, disconnecting and changing urinary equipment; applying external urinary catheters; assisting patients with toileting.

h. Implementing and/or reinforcing falls prevention strategies.


3. Collaborates with other members of the health care team by:

a. Receiving change of shift report.

b. Participating in unit in-services/meetings and unit activities.

c. Communicating pertinent information appropriately.


4. Orders, receives, checks and stocks/stores supplies as needed; contributes to routine unit inventory control.


5. Transport patients on and off units.







6. Performs various duties in support of the nursing unit, including but not limited to:

a. Answering nurse call lights/intercom and reporting patient needs to appropriate source.

b. Accurately completing documentation as required.

c. Changing linen; making unoccupied beds.

d. Checking and recording refrigerator temperatures.


7. Assists in the orientation of new employees (i.e. day-to-day implementation/application of specific job requirements).


8. Attends annual education programs to meet the Joint Commission needs.


9. Performs other functionally related duties as assigned.

Required:
HS Diploma or passage of a high school equivalency exam.
Recent graduate of a Nurse Aide or Home Health Aide Training Program, or recent experience as a STNA or Home Health Aide.
Ability to read and write in English.
Demonstrated ability to work effectively in a team environment.
Ability to communicate well with patients and staff.
Ability to maintain confidentiality.
Basic computer skills to document observations electronically.
Possess a current Basic Life Support (BLS) certification from the American Heart Association (AHA) OR obtains within 90 days of hire.

Preferred:
1-year experience working with brain injured patients/cognitively impaired patients.

Physical Requirements:
May sit, stand, stoop, bend, and ambulate intermittently during the day.
May need to sit or stand for extended periods.
See in the normal visual range with or without correction.
Hear in the normal audio range with or without correction.
Ability to lift 250-300 pounds with assistance.
Ability to push/pull up to 500 pounds with assistance.
Requires some exposure to communicable diseases or body fluids.
Requires some exposure to chemicals.
