Intake Community Health Worker

The Intake Community Health Worker (ICHW) will be the first point of contact to address the health disparities and vaccine access for communities of Color by completing a intake and assessment. ICHW will work with Capital District Latinos to provide COVID-19 Vaccine education, assist with Vaccination PODs, and refer clients to resources such as food, housing, clothing, and wrap-around services.

Full-time 

Bachelor Degree, LMSW, LCSW, Spanish a plus

Total Annual Salary & Benefits Package: $61,110.00
