St. Joseph’s Health is seeking an energic and engaging Advance Practice Provider to join our Pulmonary Office

This is an exciting opportunity to practice on a continuum of community health. By working in our primary practice setting, you are able to not only treat acute illnesses, but also develop and contribute to established treatment plans for chronic illnesses, as well as providing preventative medicine. Our new team member should have an interest in patient care across all generations and have the desire the improve our community’s health by providing a focus of care on the whole individual and access for medical care.

Our ideal candidate would have internal medicine or family practice experience or an experienced Advanced Practice Provider looking to transition to a new field. New graduates with a passion for primary care and preventive medicine may also be considered.

Education, Certification and Licensure:

  • Family Nurse Practitioner: Current New York State RN license; experience in pertinent areas of nursing practice; graduate of accredited Family Nurse Practitioner program; Master’s Degree in Nursing preferred; National certification as Nurse Practitioner or eligible.

  • Physician Assistant: Graduate of accredited Physician Assistant program, Master’s Degree preferred; National certification as Physician Assistant or eligible.

  • Currently NYS License or eligible
  • DEA Eligible

Direct Care functions include:

  • History and physical exams
  • Established appointments
  • Ordering and interpreting medications/labs/EKGs/CXRs
  • Homecare planning
  • Family education
  • Immunization scheduling
  • Creating and implementing plans of care for patient and family
  • Contributing to established care plans

  • Physical Demands:
  • Medium work/light work: must be able to lift 25 pounds occasionally. Will require ability to stand, walk, sit, bend, twist, squat and reach.

  • APP Leadership, Office Management, & Supervising and Collaborating Physician
