The QDDP performs highly responsible duties related to the development, implementation and coordination of Individual Service Plans for individuals with developmental disabilities. Maintains,reviews and evaluates pertinent program data relative to each individual's growth and development; ensures that services are being correctly implemented; provides training to direct care staff as indicated; reviews individual service plans on a monthly basis. This position is based out of Clarissa with work and travel primarily in Todd, Morrison and Wadena Counties.

The position requires at least a bachelor's degree in a human services field from an accredited college or university and a minimum of one year (full time) direct work experience with consumers with developmental disabilities. Excellent verbal/written communication skills are needed.

A valid Minnesota driver's license and clear DHS background check required. EOE
