  • Manages day-to-day operations in collaboration with the Clinical Market Manager.
  • Coordinates patients care with physician office and referral sources, ave daily census of 150 patients.
  • Collaborates with Intake staff to ensure timely admissions and eligibility for services.
  • Completes EMR workflow responsibilities to minimize delays in scheduling, order and PCR management and meet KPI benchmarks.
  • For example: OASIS, Plan of Care (485), and on-going Verbal Orders.
  • Reviews admission documentation/plan of care for appropriate visit frequencies.
  • Evaluates staffing, capacity, and volume/productivity needs.
  • Collaborates with Patient Service Coordinator with timely and appropriate deployment of services.
  • Conducts patient visits as needed.
  • Facilitates the patient experience throughout the episode of care.
  • Addresses patient complaints and elevates as needed.
  • Collaborates with Medical Record Staff for issues/delays with unsigned orders.
  • Implements and evaluates changes in processes and procedures.
  • Acts as an EMR super user to support field staff with documentation.
  • Orients and trains new staff, conducts competencies on new and tenured staff.
  • Provides age-specific clinical care as indicated.
  • Participates in after hours (nursing on call) and weekend coverage as assigned.
  • Maintains expertise in all regulatory, accrediting, licensing requirements and acts as a staff resource.
  • One year of experience in home care environment, knowledge of OASIS preferred.
  • EMR, specifically Homecare Homebase preferred.

  • ASN or BSN OR Bach/Master/Doctorate in OT, PT or Speech Therapy.
  • OH license per discipline.
  • CPR w/ 3 mths.
  • OASIS cert w/in 1 yr.
  • DL/insurance.
