Job Summary and Qualifications

LMSW completes in-depth assessments of patients in order to evaluate candidacy for solid organ transplant. LMSW provides counseling, support, and community resource referrals to aide patients/families in preparing for transplant. LMSW follows patients pre- and post-transplant to assist with resolving psychosocial issues and lifestyle changes that improve a patient’s post-transplant success. LMSW works closely with multi-disciplinary transplant team to assess inpatients in order to identify appropriate discharge plans. LMSW provides assessment and intervention planning with patients experiencing substance abuse and/or mental health issues. LMSW acts an advocate for patients that may be victims of Child Abuse, Domestic Violence, or Adult/Elder abuse. LMSW will participate in the education, training, and supervision of MSW students during internship with Transplant Social Services. LMSW will participate in community outreach and patient education related to transplant. LMSW will assist manager in policy development, problem solving, program development, and staff training. LMSW performs all other duties as assigned.

  • Completes in-depth transplant evaluations to determine a patient’s candidacy for transplant.

  • Provides evaluation and intervention for transplant patients in a crisis situation.
  • Provides short-term counseling/treatment to patients and families throughout the transplant process, adjustment to chronic illness, and/or hospitalization.
  • Collaborates with multi-disciplinary transplant team to assess and formulate appropriate discharge plans for inpatients.
  • Works to link transplant patients with Medication Assistance Programs when needed and facilitate other appropriate community referrals.
  • Executes documentation and reporting requirements.
  • Works closely with patients and staff to formulate treatment plans to resolve substance abuse, mental health, adherence, and/or psychosocial issues.
  • Completes evaluations of potential Living Donors to assess candidacy for donation and provides appropriate post-surgery follow-up for Living Donors and their families.
  • Provides educations, training, and supervision to MSW students completing their internship with Transplant Social Services. 


What qualifications you will need:

  • Licensure/Certification/Registration: LMSW required.

  • Education: Master’s in Social Work from and accredited program required
  • Experience: At least three years post-Master’s experience required, preferably in a healthcare setting.
  • Special qualifications: Must be able to successfully complete established competencies for the position within designated probationary period. Excellent skills in written and oral communication and guest relations; knowledge in human growth and development and emotional needs of children, adolescents and adults as well as the special needs of chemical dependent and/or abused clients; basic organizational, problem solving and leadership skills; skills in documentation and record-keeping; basic marketing strategies; skills in conducting individual, family, and group therapy’ skills in maintaining information as confidential; knowledge or standards of ethical and quality care.
  • Work schedule: Liver Acquisition Clinic - Full-time - Monday-Friday(8:00 am-4:30 pm)
