Role:

Under RN direction and supervision, the Hospice Aide performs personal care, homemaker assistance and defined nursing procedures for terminally ill patients and their caregivers. 

Qualifications:

  • Current Florida certificate as CNA.
  • Valid driver's license and automobile insurance in the State of Florida.
  • Reliable transportation to meet work schedule.
  • One (1) year CNA or home health aide experience (Hospice Aide Training Program can be substituted for homecare/hospice experience).
  • Ability to use equipment with visual and auditory mechanisms.
  • Ability to communicate (written and oral) in English.
  • Ability to manage the emotional stress of working with and caring for terminally ill patients and their families.
  • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation.
  • Must be able to physically access any home or facility in assigned area, i.e., navigate stairs, and narrow spaces, tolerate heat and lack of air conditioning.
  • Must be able/willing to have contact with individuals with communicable diseases.
  • Ability to physically access any home i.e. navigate stairs and narrow spaces, tolerate heat and lack of air conditioning, in any area Company serves.

 

Responsibilities:
Clinical

  • Maintain competencies/contact hours as required by HPC and state. 
  • Provide patient/caregiver services according to written Hospice Aide/Caregiver Plan of Care and HPC policies and procedure
  • Practice safe and proper body mechanics relative to moving, positioning and transferring patients.
  • Assist patient and or caregiver in maintaining a clean and safe home.
  • Provide caregiver relief as part of the plan of care.
  • Transport medications, supplies, lab specimens, documents and other items needed to meet the plan of care as requested by the RN Care Coordinator.
  • Under the direction or supervision of an RN, the Hospice Aide may perform activities that promote activities of daily living, including:
  1. Vital signs/weights
  2. Bed Bath 
  3. Sponge Bath
  4. Tub Bath 
  5. Shower
  6. Perineal care
  7. Grooming
  8. Mouth care 
  9. File Nails 
  10. Shampoo hair
  11. Shave
  12. Skin care - basic: application of lotion 
  13. Apply over-the-counter ointment 
  14. Positioning body/alignment 
  15. Transfer to bed, chair
  16. Assist with ambulation and exercises
  17. Range of motion 
  18. Ted hose application/removal
  19. Give food and fluids 
  20. Light meal preparation 
  21. Offer/empty urinal/bedpan 
  22. Empty colostomy bags 
  23. Empty foley/external catheter bags 
  24. Measure urinary output 
  25. Emotional support 
  26. Reality orientation/socialization
  27. Caregiver relief
  28. Make occupied/unoccupied bed 
  29. Light housekeeping
  30. Laundry related to patient's care
  31. Report significant changes and needs
  32. Document significant observations 
  33. Provide input into planning and evaluation of care

The Hospice Aide may not perform the following:

  1. Administer medication 
  2. Change sterile dressings 
  3. Tracheostomy care
  4. Gastric feedings 
  5. Trim fingernails/toenails
  6. Visit after hours unless pre-approved by supervisor
  7. Visit survivors unless pre-approved by supervisor
  8. Run errands that require exchange of money 
  9. Apply heat
  10. Suctioning 
  11. Catheterization
  12. Gavage/lavage
  13. Any procedure not in HA Plan of Care 
  14. Colostomy care (change wafer)
  15. Apply ointments containing prescription medication
  16. Transport patient/family

Communication

  • Contribute to the development of the Hospice Plan of Care by reporting observations to RN
  • Immediately report to the RN or supervisor any changes in patient/caregiver needs 

Other

  • Maintain medical records in accordance with HPC policies and procedures, i.e., Santrax documentation.
  • Attend HPC and mandatory scheduled meetings and participates in committees.
  • Attend HPC and community educational offerings as appropriate.*
