Registered Nurse Care Manager - Managed long-term care (MLTC) Program

Visiting Nurses Home Care seeks an experienced Registered Nurse leader in our Menands office to direct care management services for the chronic, long-term care population and coordinate the services of the MLTC program. The Registered Nurse Care Manager oversees the delivery of services, ensuring regulatory compliance, adherence to protocols and standards, quality of services, and efficient care delivery processes. $3,000 Sign-On Bonus!

Requirements:

• Bachelor of Science in Nursing or a Registered Nurse with relevant experience.

• Background in the administration and management of long-term care programs or certified home health care highly desirable.

• Experience as an Registered Nurse Care Manager preferred.

• Current NYS license and registration to practice as a Registered Professional Nurse required.
