This is a 7-on/7-off night shift position. 

POSITION SUMMARY: The Clinical Pharmacist will work under the direction of the Pharmacy Manager to maintain a high -quality, cost-effective, integrated Transition of Care program for UH Health System. To align with our Zero Harm Initiative, The Clinical Pharmacist will participate in medication reconciliation process in a system-wide to decrease medications errors and readmission rates.  Will compile a Best Possible Medication History (BPMH) for admitted patient’s, document and effectively communicate with health care providers. The Clinical Pharmacist will exercise clinical judgment to meet the needs of patients, providers, and staff consistent with UH values and mission statement. 

  • Responsible for the safe, effective, and cost effective use of medications through the application of specialized knowledge, skills, and professional judgment.
  • The pharmacist serves as a patient advocate when planning and implementing pharmaceutical care by considering the patients disease conditions, background, need for understanding and participation in drug therapy in a fiscally responsible manner.
  • These activities occur in all aspects of pharmacy practice including adult and pediatric inpatient areas, operating room pharmacies, ambulatory oncology and outpatient services ie., Bolwell Pharmacy.
  • The pharmacist is legally responsible for the review and approval of all activities related to the dispensing of patient specific pharmaceuticals pursuant to a valid prescription or medication order from a practitioner as defined in ORC 4729, 4715, 4731, and or 4741.

 

  • Essential Duties:
    • Performing a complete home medications reconciliation for newly admitted patients

    • Improve patient safety and compliance by assessing adherence to medication and patient education to reduce hospital readmission rates.

    • Work as a preceptor for PGY-1 UH Meds ambulatory residents

    • Creating and implementing projects with UH meds residency program

    • Accurate order verification and dispensing

    • Minimize order turnaround time.

    • Medication monitoring and documentation.

    • Documentation of interventions.

    • Documentation and performance of medication reconciliation.

    • Inventory control and adherence to Hospital Formulary policies.

    • Adherence to Hospital and Departmental Policies and Procedures.

  • Bachelor Degree
  • R. Ph - Pharmacist - Registered
  • Previous hospital experience with emphasis on computerized unit dose, sterile products, and patient focused clinical skills preferred.
  • Good communication skills are essential.
  • Computer and office equipment, automated compounding systems.
