  • CERTIFIED MEDICAL ASSISTANT (CMA) CLINIC LEAD - Granite Falls

    KEY RESULT AREAS:


    Patient Care:


    Assist nursing with functions related to the treatment programs in Project Turnabout.

    Assist the patients to maintain and improve their physical status.

    Assist in preparing and maintaining the health records of patients.

    Patient Care:

    Positions Available:

    Willmar, MN - Monday through Friday, four ten-hour evening shifts.

  • HUMAN SERVICE TECH - Full Time Day Shift - Granite Falls

    Patient Care Responsibilities:

    1. Attend all in-house programming with patients.

    2. Supervise patients during setup and teardown of group areas as indicated by Unit Coordinators.

    3. Help transfer patients within the facility.

    4. Clean patient units as necessary after a patient transfers/discharges.

    5. Orientation of new patients on the unit:

    a. Go over unit expectations and safety procedures.

    b. Introduce them to peers and surroundings.

    c. Document information related to orientation.

    d. Must be completed during the same HST shift as when the patient arrives on the unit.

    6. Search patient belongings when a patient arrives on the unit.

    7. Share personal experiences, wisdom and knowledge, as appropriate.

    8. Escort patients to daily activities – med passes, lunch, rec, lecture, etc.

    9. Make regular rounds of the unit and report any unusual observations to the charge nurse.

    10. Carry company-provided cell phone with while on duty.

    11. Provide patient transportation as needed.

    12. Empty trash.

    13. Overnight HSTs must remain awake and alert at all times.

    14. Laundry, as needed (overnight HST only).

    1. Complete a shift report regarding any unusual behaviors and circumstances for each patient.

    a. Email shift report to appropriate counseling staff to keep them informed of these incidents.

  • HUMAN SERVICE TECH - Granite Falls & Marshall

    Patient Care Responsibilities:

    1. Attend all in-house programming with patients.

    2. Supervise patients during setup and teardown of group areas as indicated by Unit Coordinators.

    3. Help transfer patients within the facility.

    4. Clean patient units as necessary after a patient transfers/discharges.

    5. Orientation of new patients on the unit:

    a. Go over unit expectations and safety procedures.

    b. Introduce them to peers and surroundings.

    c. Document information related to orientation.

    d. Must be completed during the same HST shift as when the patient arrives on the unit.

    6. Search patient belongings when a patient arrives on the unit.

    7. Share personal experiences, wisdom and knowledge, as appropriate.

    8. Escort patients to daily activities – med passes, lunch, rec, lecture, etc.

    9. Make regular rounds of the unit and report any unusual observations to the charge nurse.

    10. Carry company-provided cell phone with while on duty.

    11. Provide patient transportation as needed.

    12. Empty trash.

    13. Overnight HSTs must remain awake and alert at all times.

    14. Laundry, as needed (overnight HST only).

    15. After-hours snow removal, as needed.



    Administrative Responsibilities: 

    1. Complete a shift report regarding any unusual behaviors and circumstances for each patient.

    a. Email shift report to appropriate counseling staff to keep them informed of these incidents.

    2. Enter orientation and behavior notes into the patient's chart in the EMR.

    3. Document hourly rounds in EMR.

    4. Prepare documents in EMR for patient charting as directed by Unit Coordinators.

    5. Assist with training of new employees, as needed.


    PERSONAL REQUIREMENTS:

    1. Attend and participate in professional workshops, to enhance personal/professional competency.

    2. Maintain and renew First Aid and CPR certification prior to expiration.

    3. Maintain professional boundaries and be respectful to staff and patients.

    4. Good communication skills, both written and verbal.

    5. Dress and groom oneself according to the professional standards of the organization.

    6. Refrain from creating or participating in gossip of any kind.



    PHYSICAL REQUIREMENTS:

    1. Must be able to walk and stand 50% of the time on the job.

    2. Sitting at a computer terminal and talking to patients and staff about 25% of the time.

    3. Must be able to stoop, bend, kneel, reach, push, and pull.

    4. Must have good vision and hearing.

    5. Must be able to lift at least 35 pounds occasionally. 

    6. Must be able to tolerate chemical odors from cleaning supplies.


    POSITION QUALIFICATIONS:

    1. Must be 18 years of age.

    2. Must obtain First Aid/CPR certification.

    3. Must be able to document at two (2) years freedom from alcohol/ gambling use problems.  

    4. Must meet Minnesota Department of Human Services background study approval.

    5. Must comply with all MN 245G and Project Turnabout policies and procedures.

    6. Must understand basic computer functions, including email, file navigation and web-based software.

    7. Previous EHR/EMR experience preferred.

    8. Possess a willingness to learn new skills and work in a challenging environment.

    The HST is responsible for completing a shift report of unusual incidents, behaviors and circumstances for each patient to keep counseling staff informed, entering orientation and behavior notes into patient’s electronic medical record and making regular rounds of the unit.

  • PATIENT FUNDING COORDINATOR - Granite Falls

    RESPONSIBILITIES:


    Funding:

    1. Set up financial arrangements for every patient admitted prior to admittance into a program and start a financial file on each one.  Funding arrangements are not to be discussed until notification from intake person that patient will be contacting coordinator regarding payment.

    2. Contact insurance companies to verify and obtain benefits as needed.  Make sure preauthorization and referrals are completed and that updates are done as required noting all information in patient’s financial papers and on insurance flow sheet.(Change to--) in Procentive.

    3. Assure Patient Placement Authorizations are received from counties within one week of admittance, verify for accuracy, and check any insurance coverage listed.  Contact county worker to correct errors.

    4. Complete all necessary papers and receive signatures on releases, payment arrangements, etc.

    5. Complete any HEART applications as needed and/or requested by staff.

    6. Supervisor must give prior approval on any discount or low payment arrangements.

    7. Meet with extended care patients on a monthly basis and complete Monthly Fee Determination for county patients, obtaining and verifying all forms of income such as pay stubs, AFDC, etc. (delete)

    8. Collect any money that is due as per payment arrangements and write a receipt

    9. When patient is discharged, complete discharge in computer, close out file and bill out invoices, etc. as needed.

    10. Hand out all program books to patients, keeping track of assigned numbers—when give out and returned.  If not returned, send bill for cost immediately upon discharge.

    11. On a monthly basis, bill out all extended care and halfway house invoices after receiving fee determination forms. (Delete)

    12. Set up arrangements with Domestic Abuse clients, meeting with each on at intake, collect payment and writ receipts.  Make recommendations if grant dollars should be utilized and fill out appropriate paperwork.


    Record Keeping:

    1. Obtain patient profile on discharged patients, checking for completion of counselor’s information and completing remaining portion, entering information into the computer on a monthly basis (Change to--) Enter admissions, discharges and transfers into Healthland  each day.

    2. Complete DAANES forms for programs as person is discharged after receiving from counselor, checking that their information is completed and batching together at the end of the month by program and sending in to DHS by the fifth of next month.(Delete) (and then add) in computer for admits/discharges for the Willmar and Granite Falls programs.

    3. On a daily basis record in AB Book all admissions, discharges, and transfers of patients into the programs including all required statistical information.

    4. On a monthly basis, compile statistics from rosters on average length of stay, the number of admissions and discharges, and to where discharged.  Verify statistics with other business office personnel with rosters, discharge sheets and AB Book.

    5. The first of each month, prepare new rosters for all programs, totaling hours and sessions and carrying over to new month.

    6. Review patient rosters daily making sure no two people are in the same program unless they are being transferred or outpatient.

    7. Hand out all program books to patients, keeping track of assigned numbers, when given, and returned.  If not returned, send bill for cost immediately upon discharge.

    8. Keep record of Domestic Abuse clients in Willmar for each year and give Community Relations Coordinator a list of those clients with addresses for mailing list.(Delete)


    Other:

    1. Be able to fill in at front desk if needed, answering phones, greeting visitors, helping patients, doing typing, and other necessary duties during vacations, etc. as requested.

    2. Run mail through mailing machine, affix postage, and give to tech to take to post office.(Delete)


    PHYSICAL REQUIREMENTS:

    1. Must have excellent vision and hearing in order to be able to deal with facility visitors and staff and also see a computer terminal

    2. Must be able to sit for long periods of time—2 hours at a time

    3. Must be able to stand and walk.

    4. Must be able to do some stooping, bending, kneeling, reaching, pulling and pushing.

    5. Must be able to lift 20-25 lbs.
