  • Certified community health worker role will collaborate with healthcare providers in assisting patients with education and support to improve healthcare outcomes.
  • They provide intensive case management to patients and assist with access to care, barriers in obtaining necessary resources; both community and insurance based.
  • In addition, they will act as a guide in navigating transitions of care during a patients health care journey.
  • Full time days 40 hours per week.
  • Monday-Friday.
  • The shift is 8:00 AM to 4:30 PM. 
  • The position will have various location throughout the UH system.

  • High School Equivalent / GED
  • Community Health Worker Certification required.
  • Experience in community and population health management preferred.
